ASSESSMENT OF EFFICIENCY OF GESTAGENS AND ANTIGESTAGENS IN TREATMENT OF UTERINE LEIOMYOMA

In the 1st clinical group a treatment was carried out by dufaston (1h, 2x). Patients of the 2nd group took gynestril (mifepristone), 50 ml, every day.

Thickness of endometrium, expressiveness of meno-metrorrhagia, pain syndrome, algomenorrhea were reduced as a result of treatment by gestagens. Gestagens had no effect on the volume of myomatous nodules, dysuric manifestations, ultrasound picture of adenomyosis.

Meno-metrorrhagia, dysuric manifestations were drastically reduced under the influence of antigestagens. Antigestagens caused regress of both the volume of fibromatous uterus, and the sizes of dominant nodules.

The article describes the clinical observation of cystic fibrosis in a child 4 years of age with severe cystic fibrosis mutation in the transmembrane regulator (711+1G>A). In the literature review presents current data on classification, clinical manifestations, diagnosis, and treatment algorithms. Particular attention is paid to the necessity of the  cystic fibrosis exclusion in sickly children.

Aggressive animals are distinctive by higher reproductive ability of remembering in open space where the high rate of memorization plays the great role In this particular case in Morris water maze aggressive animals develop better navigation learning than non aggressive ones. This kind of mental map could be perceived by the rat or not. In this case, this chart is well perceived by both aggressive animals. The main mechanism of remembering this kind of invisible platform could be relate to the development of proprio-vestibular apparatus.  It seems that aggressive animals remember the motion sequence  that enables them to find a target. (praxis or idiopathic strategy). This increases consolidation with spatial memory. 

Thus we can conclude that an aggressive rat, whether it has more or less anxiety, tries to perceive the environment and select safe condition that is useful to it, what is contributed by features that are characterized for aggressiveness which is manifested by working capacity. Also in open field we have seen that both aggressive animals are characterized by a good ability to perceive the environment, the duration of grooming, that helps them to make a correct decision, also set of search elements, that is characterized only for them.

Excessive expression of HER2 is seen in 20-30% of mammary or ovarian cancer/carcinomas and is associated  with unreliable prognosis. Over-expression of HER2 is very frequent at prostate carcinoma and anti-HER2 targeted therapy (Herceptin) can be used in a large proportion of patients.

The majority of clinical studies applying Herceptin monotherapy at prostate carcinoma didn’t prove its effectiveness, however, it appeared to be effective in combination with taxines.

THE SPREAD OF HCV AMONG THE SEAFARERS

Hepatitis C is great public health problem all over the world. Hepatitis C virus causes acute or chronic liver disease. Naturally most cases of acute HCV infection are asymptomatic. Approximately 15% of patients acutely infected with HCV lose virologic markers for HCV. Thus, approximately 85% of newly infected patients remain viremic and may develop chronic liver diseases. Estimated 170 million people are chronically infected with the hepatitis C virus (HCV) worldwide. At least 70% of patients who contract HCV develop chronic hepatitis C, with 20-50% of these patients eventually progressing to cirrhosis and 1-2% developing hepatocellular carcinoma in 10-20 years.

APPLICATION OF NANOPARAFFIN TECHNOLOGY IN THE PROCESSING OF HISTOLOGICAL MATERIAL

Introduction: The pathological examination is one of the longest in the list of medical tests. Most of this time is spent on preparation of the microslide. Our goal was to have developed optimal standarts of processing by applying Nano composites.

Material and methods: 8 various types and concentration nanocomposite paraffin (nanoparaffin) were created of ultrasound supported by UP200HT device on the base of Paraffin. Carried out 96 experiments with different time intervals.192 postoperative and autopsy tissue samples were used in the examination. The quality of slide were avaluted using 5 point score system by independent pathologists in blind methods.

Results: Nanoparaffin reduces standard time of keeping tissue samples in paraffin twice.  Nanoparaffin  gives chance to erase chemical agent – xylene. Nanoparaffin brings significant economic impact. The results of this project highlights that the time is spent on preparation of the slide 2,5 times decreased and 3 times decreased reagents using.

Conclusions: By reducing these two factors – time and agents spent, the cost-effectiveness of pathological examination is significantly increase.

THE ROLE OF HEREDITARY FACTORS IN PATHOGENESIS OF MYOMA UTERI

The research goal was to determine a possible role of genetic factors in the genesis of myoma uteri. The following clinical-genetic methods were applied: population-epidemiological, clinical-genealogical, genetic-correlative analysis, segregation analysis. Population and family frequencies of myoma, index of phenotypic correlation and index of heredity were determined. 17 risk-factors and index of segregation frequency were revealed, too.

USE DIODE LASER IN ENT PRACTICE

The Cerelas E diode laser was used to successfully treat 20 patients presenting with hyperplasia of the nasal turbinates within a period of approximately four months. When applied systematically, a laser output of 12-14 W at a wavelength of 980 nm is sufficient to achieve a defined ablation of the mucosal tissue. The procedure is painless and normally involves no loss of blood.

Analysis of the results indicates that in the first phase of orthodontic treatment lysozyme level in both groups was in the range of acceptable values. The concentration of lysozyme in saliva normally is in the range of 2÷15 mcg/ml.

THE RESULTS OF CLINICAL AND LABORATORY STUDIES AT LOCALIZED PROSTATE ADENOCARCINOMA

The results obtained after clinical and laboratory examinations of 68 patients with localized prostate carcinoma have been studied. In addition to the clinical data of the patient, the scores of  IPSS questionnaire, quality of life, sexual function, prostate volume, localization of hypoechogenic sites, urofluorometric indexes, results of examination of urinary organs, PSA level, ESR and blood count data have been  assessed at localized prostate cancer.

The most rapid population growth in the world is that of the last century and a half the planet’s population is the fastest growth rate. If 1 billion people living on Earth in 1850, 1930, amounted to 2 billion. The growth of the world’s population in the next decade will be even more accelerated. From 1930 to 1975 years, world population has doubled to 4 billion. Experts believe that the world population in 2025 will reach 8 billion a year. Then, when the world’s population, most of the rapid growth of the problem, the problem is quite opposite in Georgia. People-the Population Fund, the United Nations, according to the survey, 28% of the population to drop by 2050.

LASER TONSILLOTOMY OR TONSILLECTOMY?

Laser tonsillotomy represents a useful, less invasive alternative to tonsillectomy as a treatment for symptomatic tonsillar hyperplasia and chronic recurring tonsillitis in children, and is associated with greatly reduced risks. The main characteristics that distinguish this procedure from tonsillectomy are a significantly reduced risk of secondary bleeding, a shorter recovery, reduced pain and the fact that it can be performed on an out-patient basis. An additional benefit is constituted by the fact that the procedure does not remove the tonsillar parenchyma, which may still be immunocompetent.

The data obtained suggest that the level decrease in humoral immunity is one of the conditions for the development of inflammation in the periodontal tissues on the stages of orthodontic treatment.

APPLICATION OF SAFOCID FOR THE TREATMENT OF UROGENITAL INFECTIONS

Combination drug safocid was used in 4 clinical groups, which involve different combinations of urogenital trichomoniasis, chlamydiosis, mycoplasmosis, bacterial vaginosis, candidamycosis. For comparative analysis a treatment was conducted in the same groups using traditional antimicrobial agents.

Safocid manifested high antimicrobial efficiency. Clinical and microbiological efficiency was achieved roughly in 95%.

Among special advantages of safocid are following: high efficiency, monotherapeutic profile, simple application.

The epidemiology of dental caries has rich history, but fluoride concentration in drinking water and its correlation with dental caries scientifically were still reported at the beginning of the last century. Dean’s classical studies in USA provided the basis for generally accepted rule that protection from dental caries and minimal dental fluorosis are associated with different levels of  fluoride in drinking water. Pubmed Central, Medline, Embase were searched to review scientific literature in this filed. According to this data, we can conclude that water fluoride concentration  has strong relationship in prevention of dental caries.

OTOSCLEROSIS SURGERY UNDER STAPES POSITIONAL VARIATIONS INTO THE MIDDLE EAR

Variances of the stapes-bone location in the middle ear are listed that troubling the course of otosclerosis surgery. The sharp vertical position of the stapes arc over the oval window has been declared as the leading hindrance among. Under this situation, the visualization of the stapes footplate is limited or is even blocked totally that complicates the drilling of the plate and the replacement of the stapes by the prosthesis. The method is elaborated for avoiding the difficulties indicated. At the beginning of the surgery, the niche over the footplate is shaped by the 0.6-mm diameter Sciter bore just in the middle third of the rear stapes arc leg. The established space provides the rational observing of the stapes footplate and the trouble-free approach to. As a result, the stapes footplate is ordinarily drilled by the perforator while afterwards the distal tip of the stapes prosthesis is also ordinarily imbedded in the aperture created. The elaborated procedure prevents the origination of the swimming footplate that appearing a dramatic complication of the stapes surgery. Under usual stapes configuration into the middle-ear, the otosclerosis operation is advised thus to perform via the standard Fish approach. Under strict vertical position of the stapes arc over the oval window, for an adequate visualization of the surgery field as well as for fair manipulations within, the niche is recommended to bore in the rear arc leg. 67 otosclerosis operations have been executed at Geogian National Centre of Otorhino­laryngology in 2013. In 59 cases from, 88.1%, the Fish technique has been applied. In remainder eight patients, 11.9%, because of restricted visualization of the surgery area due to the sharp vertical position of the stapes over the oval window, the watching/contacting hole has been preliminary drilled within the middle third of the rear arc leg.

THE ROLE OF INDUCTORS OF TEMPORARY REGRESSION IN THE TREATMENT OF MYOMA UTERI

The contingent under investigation was divided into 3 groups: patients of 1st group were treated by antigestagens; patients of 2nd group took analogues of gonadoliberins (GnRG), and 3rd group was served as the control.

Antigestagens and analogues of GnRG are characterized by sharp fall in frequency of specific symptoms of myoma uteri, but in the 2nd group are emerged the symptoms characteristic for estrogen-deficit states.

Analogues of GnRG, in comparison with antigestagens in a less degree have regressive effect on the volume of myomatous uteri and in a greater degree – on sizes of dominant nodules.

Antigestagens cause only hypoprogesteronemia in the treatment process, while analogues of GnRG give rise to hypoestrogenemia.

OPTIMIZATION OF TREATMENT OF THREATENED MISCARRIAGE IN CASES

OF RETROCHORIAL HEMATOMA

Among 47 pregnant women with retrochorial hematoma (verified by ultrasound scanning) different hypercoagulative disturbances have been observed in 32 pregnants. In basic group (20 pregnants) the combination treatment (hormones, anticoagulant, enzyme) was used, while in control group (12 pregnants) only hormonal therapy was applied. In case of retrochorial hematoma accompanied by hypercoagulative disturbances it is preferable to apply combined therapy (dufaston, clexane, wobenzym) with the end of minimization of reproductive losses.

THE RESULTS OF MORPHOLOGICAL STUDY, INVASION DATA AND THEIR CORRELATION WITH PROGNOSTIC FACTORS AT LOCALIZED PROSTATE CANCER

The results of morphological studies obtained in 68 patients with localized prostate cancer  have been studied. The number of biopsy material, Gleason score/sum,  correlation of Gleason score with biopsy and postoperative material, correlation of Gleason score with prognostic factors have been evaluated. The data of invasion in postoperative material and rate of concommitant pathologies at localized prostate cancer were studied as well.

STOMATOLOGICAL STATUS OF ORAL MUCOSA AND ITS TREATMENT IN DIAGNOSED WITH MYELOPROLIFERATIVE DISORDERS

The work shows the results of the clinical use of drug “Camelyn” in treatment of oral mucosal lesions in patients with various forms of leukemia. 14 patients have been observed; 9 of them were previously diagnosed with acute myeloid leukemia (AML) and 5 of them with chronic myeloid leukemia (CML). The oral manifestations of leukemia were apthous stomatitis, gingivitis, keratitis, etc.  “Camelyn” ointment (5%) has been applied on oral mucosa for 10-15 minutes as a single agent and also in combination with other drugs. During the treatment decrease of pain, fast resolving of inflammation, and acceleration of mucosal regenerationhave been observed.

 The results confirm advantages of using drug in patients diagnosed with leukemia as a single agent, as well as in combination with other drugs.

SEVERE SEPSIS, SEPTIC SHOCK AND MODS IN INFANTS: EFFICACY OF PLASMAPHERESIS

Objective: Determination of relative risk of mortality, changes in organ disfunction and ICU Hospitalization length in patients who received plasmapheresis with standard treatment of sepsis, in comparison to patients who received standard treatment alone. 

Design: Prospective, randomized controlled clinical trial.

Setting: Intensive Care Unit at Pediatric Clinic of Tbilisi State Medical University.

Patients: Surgical Infant patients with severe sepsis, septic shock and MODS

Interventions: 18 patients randomized to receive either standard sepsis treatment or an add-on treatment with plasmapheresis.

Measurements: Primary endpoint was intrahospital survival. Mortality rates were detected in plasmapheresis and control groups and relative risk of mortality associated with plasmapheresis were calculated. Differences were considered statistically significant at P values less than 0.05. For measurement of changes in organ disfunctions etermination of PELOD scores and ΔPELOD before and after plasmapheresis procedure (in 24 hours) and in control group at the time of diagnosis estabilishment and after 4 days of standard treatment was done. ICU Hospitalization length in days and statistical significance of results were measured also.

Results: Relative risk of mortality in plasmapheresis group was 0,18 (95% CI 0,27-1,17) among surgical neonates with severe sepsis and MODS without septic shock, absolute risk reduction 57,5% (p=0,025). Reduction of organ systems dysfunction severity in Infants measured by PELOD score was 62% (p=0,002). Reduction of ICU hospitalization length is emphasized and statiticaly significant also.

Conclusions: Plasmapheresis could decrease mortality among surgical infants with severe sepsis, as well as severity of MODS and ICU Hospitalization length.

CONTEMPORARY PROBLEMS OF SOCIAL PROTECTION OF THE ELDERLY POPULATION OF AJARA

The paper present data about  the problems that have arisen in recent years in the field of social welfare of elderly, associated with the growth of this part of Ajarian population.

In the new economic conditions of particular importance is the question of financing the ever-increasing needs of the elderly, especially single that requires additional measures.

This is confirmed by the results of the study during which found that this contingent requires significantly greater financial resources than older people living in the family and get enough help from relatives. According to the author present situation calls for additional institutional arrangements.

ANALYSIS OF IMMUNOHISTOCHEMICAL STUDY OF LOCALIZED AND UNLOCALIZED PROSTATE CANCER

According to the results of immunohistochemical study HER-2/neu was not revealed in 69.1% of patients with localized cancer and in 18.4% of patients with unlocalized cancer, respectively. Immunohistochemical study of biopsy material and application of HER-2/neu antibody was recommended since their positivity is associated with capsular invasion and extracapsular distribution of the tumor.

AERD - ASPIRIN EXACERBATED RESPIRATORY DISEASE

AERD (Aspirin Exacerbated Respiratory Disease) is a clinical syndrome observed in 5-10 % of patients with asthma and characterized by history of acute dyspnea usually accompanied by nasal symptoms (rhinorrhoea, nasal congestion). AERD is associated with increased risk for severe asthma, frequent exacerbations. The mechanism of hypersensitivity to ASA and NSAIDs in asthmatic patients is not immunological.

In order to establish the degree of unbearability to aspirin in patient, the rate of sensitiveness toward aspirin is determined. At the time of law degree of sensitiveness (ASR to 2.0) the light AERD is developed, at the time of medium degree of sensitiveness (ASR from 2.1 to 3.0) the AERD of medium heaviness is developed and when ASR is more than 3.0, frequently occurs the heavy disease. Eliminative therapy has the leading role in prophylactic and treatment of AERD e.i. such patients should not receive aspirin, medicines of pyrazolon class (Baralgyn, Analgyn, Amidopyrin) and any tablets with yellow cover as well because they contain paint Tartrazin.

AN IMPORTANCE OF EXTRACORPOREAL USAGE OF MONOCHROMATIC

The aim of this research was to study changes of erythrocytes while extracorporeal and general radiation of blood.  For this purpose 1 ml blood was taken from normal weight, (1200 gr) grown up shinshila rabbits, that we divided into 9 groups and irradiated with 10 vat helium-neon laser during 0.5-1 minutes. After irradiation blood was injected back to the organism of rabbits. After 2-6 hours from irradiation blood was taken from veins, to study by electronic microscope.

The extracorporeal radiation, performed by laser and input of radiated blood back to organism is considered to be a shock therapy from the side of erythrocytes, which promote

the defense function of the organism itself.

The privilege of usage of extracorporeal laser, to compare to general radiation, is expressed in its ability to exclude direct simultaneous impact on the different systems of body, especially on nervous system and at the same time to provide its direct importance, at the level of specific system, tissue and cell.

MODERN ASPECTS OF UNCLOMPLICATED UROGENITAL CHLAMYDIA AMONG WOMEN OF REPRODUCTIVE AGE AND THEIR PHARMACOECONOMIC ASSESSMENT

Urogenital chlamydia is one of the most common and socially significant diseases among sexually transmitted infections, which essentially affect reproductive health of the population.

The purpose of the study was to assess therapeutic effectiveness of antibiotics – Azithromycin and Doxycycline, compliancy of their use and their pharmacoeconomics for treatment of uncomplicated chlamydia among women of reproductive age. 

Sixty-seven women of reproductive age participated in the study who were diagnosed with urogenital chlamydia in 2013-2015.

The conducted studies suggest that both medicines: Azithromycin and Doxycycline are equivalent for treatment of uncomplicated chlamydia, although nowadays it is important to determine costs for treatment i.e. their pharmacoeconomics. The principle – balanced clinical and economic results is observed in case of treatment with Doxycycline, which is profitable for patients, clinics and also insurance companies.

PARTICULARITY OF THE BRANCHES OF ABDOMINAL PART OF THE VAGUS NERVE

There is studied the branches of the abdominal part of the vagus nerve by anatomical dissection method.

There was designated variants of the branches of the anterior and posterior trunk of the vagus nerve.

There is determined the division of the levels of the vagus nerve.

The results has shown us, that damage of antral nerves occurs when in development of these nerves takes part all or any of this trunk, antral nerves does not pass into the lesser omentum, but it descends on anterior or posterior wall of the stomach.

APPLICATION OF OINTMENT CYCLOFERON IN COMPLEX TREATMENT

In reproductive aged woman administration of drugs may cause the suppression of innate immune system. It causes suppression of vaginal biotipe and growth colonies of pathogenic microorganisms.

The purpose of our research is to estimate the effectiveness of comlex treatment of genital candidiasis. For this reason 30 woman of reproductive age were observed. They were treated by antifungal therapy in comlex with Ciсloferon-5% liniment application. After the treatment it was stated that the number of remission was significantly decreased.

We have briefly described the nature of theories and their relationship and uses to practice. the research process requires us to engage at some stage with theoretical perspectives. Of the different theoretical perspectives available, positivism and various strands (post-positivsm, critical theory and others)  are among the most influential. They illustrate the value of adopting theoretical perspectives that are congruent with the researcher’s epistemology and demonstrate the kinds of research methodologies that emerge from them.

METABOLIC  SYNDROME AS  A RISK-FACTOR  FOR  PREGNANCY

Metabolic syndrome and  its   components  worsen  progress of pregnancy. For this reason 72 pregnant woman in the age 21-40  with obesity have been observed, according to the special medical forms, which contained BMI (Body mass index), ABP (Arterial blood presure) and waist circumference.  It was possible to reveal the main risk-factors of metabolic syndromes during the period of pregnancy and possibilities of their timely correlation.

MODERN VISION OF DIAGNOSIS OF GYNECOLOGICAL DISORDERS

Over the last two decades molecular genetic studies of gynecologic malignancies, especially malignant tumors, have contributed to fundamental understanding of their molecular pathogenesis. Molecular studies have recently focused on diagnosis field. This chapter provides review of the molecular markers for the most common genital tract tumors that are currently used in clinical practice for diagnosis, prognosis of the most common gynecologic malignancies and assessment of their therapeutic effects.

The following methods belong to molecular pathology detection techniques: immunohistochemical evaluation that identifies antigens (proteins) or phenotype expressed in cell cytoplasm and/or cell nucleus  and in situ hybridization (ISH) technique that is used for identification of DNA/RNM in cells i.e. genome (genotype) changes. 

Pap test is used to diagnose and monitor cervical pathologies, although in case of high-grade squamous intraepithelial lesion (HSIL) i.e. CIN-2-3 proliferation marker Ki67 is used and Cish method is applied for detection of HPV high-risk types.

Molecular pathological approaches contribute significantly to improving our knowledge as well as and quality of detection, also selecting and using effective treatment methods that is a step taken forward in medicine.
