THE INFLUENCE OF TRUNCAL SUBDIAPHRAGMATIC VAGOTOMY AND LIGATION OF THE GASTRIC BLOOD VESSELS ON THE PORTAL PRESSURE IN EXPERIMENT

TSMU, Department of  Topographic Anatomy, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 9-11.

Experiments were performed on 30 adult male and female dogs, with body weight of 8 to 20 kg. The animals were distributed in 3 groups. The I group was a control group (6 animals). In the II group the truncal subdiaphragmatic vagotomy was performed in 12 dogs. In the animals of the III group the truncal subdiaphragmatic vagotomy was combined with the ligation of  the left gastric artery and vein. In all the experimental groups, before the operation, and 10 min., 2 hours, 3, 9 and 14 days after the operation, the portal pressure was measured.

It was shown that truncal subdiaphragmatic vagotomy combined with the ligation of  the left gastric  artery and vein results in significant and stable  decrease of the portal pressure. The pressure remains at a low level during the whole studied postoperative period. Such a decrease of the   pressure in the portal system, together with the inhibition of proteolitic activity and  total acidity of the gastric juice, can have the haemostatic effect during gastroduodenal bleeding in case of the acute gastric and duodenal mucosal ulcers.

MORPHOLOGICAL CHARACTERISTICS OF GINGIVAL MUCOUS CELLULAR COMPOSITION IN EXPERIMENTAL GINGIVITIS

A.Natishvili Institute of Experimental Morphology of Georgian AS, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 15-18.

The aim of the work was to estimate morphological changes in cells of gingival mucous membrane in the experimental gingivitis model.

Fifty rats with the body weight of 120-160 gr were used in the experiment. Gingivitis was modeled by front tecte ligation after a preliminary exfoliation of the mucous membrane in the dental neck area. The specimens were examined by histological method and electron microscopy on the 3rd, 5th, 14th and 28th days of the experiment.

It has been shown that cellular population of plasmocytes, lysphocytes, fibroblasts and mast cells take part in the inflammatory process.

In all the areas of microcirculation, transformation of endotheliocytes like papillas occurs; the process of roughening of collagenous fibrils indicates their significant role in the inflammatory damage of gingiva.

Reaction of biological amines TO THE toxic action of phenol IN the experiment

A.Natishvili Institute of Experimental Morphology of Georgian AS, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 18-21.

The goal of the experimental research is definition of the alterations of hypothalamus biological amines concentration during intoxication with phenol vapor.

The research was held on 40 grown-up white male rats with 200-250 gr of mass. Each animal received phenol vapor inhalations in the special closed chamber for 15 days.

The animals were researched by decapitation on the 16th, 30th, 45th and 60th days after the experiment had started. By means of high pressure chromatography method we defined concentration of biologic amines: serotonin, dopamine, and noradrenalin in the hypothalamus. All the primary data were considered to be 100%. The research showed that the concentration of noradrenalin on the 16th day reduced to 76%, by  the 30th day it remained 76%, by the 45th day it began to increase up to 82%, and on the 60th day of the experiment it increased as compared to the norm and made 111%. Definite regularity appeared against concentration of dophamine. On the 16th day it increased to 148% and by the 30th day decreased to 55%, by the 45th day it again began to increase and reached 61%, and on the 60th day surpassed the norm and made 128%. There are alterations in concentration of serotonin as well. Namely, for the 16th day after the beginning of the experiment it reduced to 80%, on the 30th day it continued to lessen to 74%, on the 45th day it began to increase and reached 82%, and on the 60th day made 83%. These data are different from the above considered alterations of noradrenalin and dopamine concentrations.

Reasoning from the above mentioned intoxication with phenol vapor in the organisms of male rats causes definite alterations that are expressed in the changes of biological amines concentration in hypothalamus.

TECHNOLOGY AND EFFICIENCY OF   LOW-FLOW ANESTHESIA

IN ELDERLY AND SENILE PATIENTS

Department of Anesthesiology, Critical Medicine and Toxicology, Georgian State Medical Academy, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 24-28.

The study was undertaken to evaluate the efficiency and safety of low-flow anesthesia in elderly and senile patients. The parameters of circulation, respiration, microclimate in the breathing circuit and recovery after anesthesia were explored in 76 patients (ASA II-III), aged 65 – 84 years, underwent abdominal surgery.

Examining of parameters of respiration and circulation indicated the advantages of low-flow anesthesia. It was demonstrated, that the Low-Flow anesthesia is an effective method for ensuring on adequate gas exchange and steady-state values of blood circulation in elderly and senile patients. The authors found previously unknown parallels between the Ppeak and the level of fresh gas flow in the breathing circuit. The decrease of humor and heat loss from the patients’ airway, create physiological condition in the breathing circuit. Early awakening allows early activation after abdominal surgery.

J. Experimental and Clinical Medicine, 2006, 3, pp. 28-30.

Indicators of the IgG-antibody concentrations ratio in the blood serum of  children with cerebral palsy reflect immune system reactions before and after the use of the device “Gravistat” more accurately than indicators of the concentrations of autoantibodies against the separate neuroscpecific protein, and in 68% of cases correspond with a degree of the disease treatment efficacy which becomes evident under the effect of the method of Dynamic proprioceptive correction in 4-6 weeks. The prognosis and monitoring of therapy for cerebral palsy in children via the “Gravistat” device can be a practical application of the worked out test.

THE ROLE OF COMPUTER TOMOGRAPHY IN DIAGNOSIS OF LUNG TUBERCULOSIS AND IN DIFFERENTIAL DIAGNOSIS

Institute of Medical Radiology, Georgian AS; National Tuberculosis and Lung Diseases Center;

State Medical Academy of Georgia; Tbilisi Clinical Hospital N4, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 32-36.

The purpose of the study was to determine CT indices for hematogenic, lymphogenic, and bronchogenic spread in disseminated lung tuberculosis, evaluate a type of this disease and assess the clinical importance of the date obtained.  50 patients (34 males and 16 females, age-14-73 years) with clinical signs of the lung tuberculosis underwent radiographic and SPCT examinations, dynamic clinical and radiological supervision.  The majority of these cases (86%) showed mixed genesis, but mainly hematogenic spread was found in 44%, predominance of lymphogenic spread took place in 30%, and bronchogenic-in 26%. A high frequency of adenophathy was accompanied by interstitial and primary forms.  The study showed a high sensitivity of SPCT in detecting the activity, genesis and widespread of all these forms, especially for interstitial damage. It was discovered in clinically suspected cases when the chest radiogram was normal or unconvincing.  The definition of the morphological structure of the damage is much more precise on SPCT.   The determination of genesis and activity of TB disseminations controls treatment, raises reliability of prognosis. This is most important for avoiding different complications.

ENDOTHELIAL DYSFUNCTION AND HYPERLIPOPEROXIDEMIA IN PATIENTS WITH CORONARY ATHEROSCLEROSIS. PLEIOTROPIC EFFECTS OF CARDIRAT

Emergency Cardiology Center, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 39-42

In 48 patients with coronary atherosclerosis endothelial dysfunction and hyperlipoperoxidemia were observed. Mean plasma concentration of nitric oxide (NO) was reduced -10,8±2,53 mkmol/l. In contrast, mean plasma lipoperoxide (LPO) level was increased -3.27±0.48 mkmol/l. On the background of 24.8±8.1 mg/d Cardirat (Carvedilol) treatment there were statistically significant changes in plasma concentrations of NO and LPO. The obtained results of this trial indicate that endothelial dysfunction and hyperlipoperoxidemia are ordinary events in coronary atherosclerosis. In such a category of patients Я-blocker cardirat revealed antioxidative efficacy by reducing the degree of  oxidative stress and restoring of endothelial function. Consequently, the present study supports the use of Cardirat in patients with coronary atherosclerosis in order to prevent recurrent coronary events.

DIAGNOSIS OF GROWTH HORMONE DEFICIENCY IN CHILDREN WITH IMPAIRED GROWTH

Clinic “ENMEDIC”, Chair of Endocrinology of the Georgian State Medical Academy

Chair of Endicrinology of the Tbilisi State Medical University

Hamburg Univesity Children’s Hospital, Department of Endocrinology

J. Experimental and Clinical Medicine, 2006, 3, pp. 45-48.

Growth hormone deficiency (GHD) as a syndrome comprises multiple pathogenetically distinct entities that are caused either by impaired growth hormone secretion or by impaired peripheral action of GH. The diagnosis of GHD in a child with impaired growth cannot be made on the basis of auxologic criteria alone, since these are similar for a variety of growth disorders arising from different causes. The aim of the study was to evaluate the results of different diagnostic tools for children with impaired growth and establish the diagnosis of GHD among them.

We have investigated 32 children (30 males and 2 females). All of them underwent insulin tolerance test (ITT), GH was determined in duplicate by RIA (Seria) and IFA (DSL). The part of children with peak GH concentration in ITT<10ng/ml  were retested with Arginine tolerance (ATT) test. The concentration of insulin-like growth factor-1 (IGF-1) and free thyroxin (fT4) in serum was determined in all patients.

Serum IGF-1 levels, as well as height velocity (HV) and the relation of bone age (BA) to chronological age (BA /CA) was significantly low in children with GHD. Some children with peak GH concentration in ITT<10ng/ml had developed normal response in ATT. Our study demonstrates that the best strategy for diagnosis of GHD is combination of laboratory and clinical investigations.

CLINICAL DATA OF CERBROLYSIN ADMINISTRATION IN ANTIPARKINSON COMPLEX THERAPHY

Neurology Department of Tbilisi State Medical University, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 51-53.

The aim of the study was to determine effectiveness of Cerebrolysin as an additional medication in complex treatment of Parkinsonism. We studied 22 patients (62-75 years old) with Parkinsonism, which underwent specific monotherapy or complex treatment by L-Dopa, MAO-B/ comt inhibition, DA-Agonists for 1-5 years. Decreasing of efectiveness of antiparkinson therapy, and consecutive worsening of the patients’ condition were the indication criteria for the onset of treatment by Cerebrolysin. Cerebrolysin was prescribed as 5-10ml i.v. injection for 10 days each month. Adding Cerebrolisin to antiparkinson treatment decreased more oligobradikinesia than tremor. In all cases the main parkinson treatment scheme was not changed.

Including Cerebrolysin into antiparkinson treatment increases the effectiveness of main therapy, enables us to avoid  in dose increasement, improves patient’s quality of  life, better affects  oligokinesia than tremor.

HUMAN PAPILLOMAVIRUS AND CERVICAL INTRAEPITHELIAL NEOPLASIA

I.Zhordania Institute of Human Reproduction, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 53-56.

Human papillomavirus (HPV) infection is the most common sexually transmitted disease in the world. Human papillomaviruses are associated with a spectrum of diseases. ranging from common warts to invasive carcinoma of the genital tract. The clinical manifestations of HPV-infection depend on the viral subtype, the immune status of the patient and environmental co-carcinogens.

There are known more than 100 subtypes of HPV, 20 out of them are oncogenic viruses: low-risk types are HPV 6,11,42,43,44, associated with low grade dysplasia; intermediate-risk types-HPV 31,33,35,52,58 and high-risk types-HPV 16,18,45,56 associated with high grade dysplasia and carcinoma.

The aim of our study was to detect HPV and cervical intraepithelial neoplasia (CIN) in the patients with condylomatosis by colposcopy and Pap-smear test.

INFLUENCE OF PLAFERON LB ON THE  SNP-INDUCED APOPTOSIS IN JURKAT CELL CULTURE

A. Natishvili Institute of Experimental Morphology of Georgian Academy of Sciences

Tbilisi State Medical University, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 56-58.

The aim of the study was establish Plaferon LB influence on NO-induced apoptosis in Jurkat cell culture.

Apoptosis was induced by incubation of cells with natrium nitropruside (SNP) in dose 1 mM during 24 and 48 hours. To the part of cells was simultaneously added Plaferon LB by dose  4 μM/ml. In the cell culture the mitochondrial potential (Δψ), distribution of cells between cell cycle phases and apoptosis intensity were investigated by flow citometry method and using fluorescent probe DiOC6 and propodium iodide.

By the results of the experiments it was established that SNP stimulates apoptosis and suppresses proliferation (mitosis) in human adult T cell leukemia (Jurkat) cells, but doesn’t influence  the population of diploid cells in phase G0/G1.Plaferon LB provides reduction of the apoptosis intensity, restores mitochondrial potential and promotes intensification of cell proliferation, that revealed by its mitosis-stimulating activity.

In 70 cases (48,9%) out of 143 was found in abnormal transformation zone, in 67 cases (46,8%) - aceto-white epithelium, in 13 cases (9,1%) - mosiac, in 20 cases (14%) - punctuation. In 93 patients (65%) was found iod-negative zone, in 9 cases (6,3%) - leukoplakia.

According to our results HPV-lesions were found in 98 women (68,5%) and CIN was diagnosed in 30 patients (21%): 15 women had CIN 1(10,5%), 12 women- CIN 2 (8,4%) and 3 women- CIN 2 (8,4%).

ACUTE APHATIC SYNDROME DUE TO CEREBRAL SUBCORTICAL STRUCTURES DAMAGE

Neurology Department of Tbilisi State Medical University, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 58-61.

We reviewed 24 cases of subcortical aphasia (10 due to ishcemic stroke and 14 due to intracerebral hemorrhage) without the presence of cortical damage. The difference in the structure of aphatic syndromes cases allows us to make conclusion  that secondary cortical hypoperfusion is not the only mechanism of aphasia. We hypothesized that diaschisis is another possible reason for acute aphasia in these cases. We also can not exclude the possibility that subcortical lesions can directly cause language deficits.

EXPERIMENTAL VALID OF STIMULATING ROLE OF HYDROXYAPATITE  AND PLATELET  MASS IN REPARATIVE  OSTEOGENESIS

I.Javakhishvili State University of Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 64-67.

It’s a common knowledge that calcium phosphorus are the basic elements in making the inorganic matrix of bone tissue. The purpose of our research was to study the local effect of hydoxypatite and platelet mass on the course of reparative osteogenesis. We used hydroxyapatite as the source of calcium.

The research showed that in the laboratory group at the initial stage of differentiation from the moment of traumatization permanently appears the pre-osteoblast and osteoblast contour  . And in control group the resorption processes were clearly expressed. At the end of the 50 day experiment  in the experimental group the compaction of osteogenic structure was observed in all defect areas. Newly born bone suffers compaction. And in control group  only the defect filled with bone cancellous tissue.

The results of the experiment prove that the hydroxypatite has a stimulative role in the process of osteogenesis. Activation of the reparative processes is far more intensive at the final stage of osteogenesis and with the direct participation of  hydroxyapatite and platelet mass in the processes  of calcification of organic matrix.

I.Paghava Pediatric Scientific Research Institute;

K.Chachava Research Institute of Perinatal Medicine, Obstetrics and Gynecology, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 73-76.

The aim of the work represented the estimation of influence of the early ontogenesis factors on the physical development rates and the state of the health in the children of primary school age.

The research covered random selected 110 children of primary school age (aged 7). The health state of mother and child was estimated on the different stages of the ontogenesis. The data of the masse and height of the children at the moment of the birth were considered, the somatic health and physical development of the children of primary school age ware studied.

It was ascertained under the received results, that the mass and height of the newly born baby was decreased in the contingent where the different pathologies ware noticed in mothers during the pregnancy in comparison with the group of the mothers who did not have any pathologies during the pregnancy. The growth and development statistic changes were noticed in the girls, whose mothers were ill during pregnancy, during few months after the birth as well as at the age of 7. At the same time, the illness of mothers during pregnancy correlating to the frequency of the abnormality of the children health according to all the studied parameters and especially in girls. So, the one of the important reasons for the worsening the child’s health might be increasing illness of the pregnant women that mostly is transmitting in female gender.

TREATMENT OF CHRONIC URETHRITIS AND PROSTATOVESICULITIS IN INFERTILE MEN BY PHYSIOTHERAPEUTIC METHOD

Andrology Department of the Research Institute of Dermatology and Venerology

J. Experimental and Clinical Medicine, 2006, 3, pp. 76-79.

60 patients with chronic urethroprostatitis and prostatovesiculitis were treated by physiotherapeutic means – the “Intramag” apparatus, and 19 men constituting a control group were treated by a traditional method - antibiotics+massage.

After the treatment, 43(71,7%) patients of the study group recovered clinically as well as etiologically, and in the control group, fully recovered 6(31,6%) men.

A highly effective treatment with the “Intramag” apparatus of patients with chronic inflammatory diseases of the prostate gland, testes, urethra, with disorders of reproductive function is provided by the complex impact along the full length of the urethral mucosa of the following factors: micromassage, irrigation and ablution with medicinal solution, magnitophoresis and electrophoresis of the drug into the urethra as well as the prostate. The device allows reducing treatment terms by 3-4 times as compared with the well-known ablution and instillation methods, reducing them in a series of cases to 3-6 sйances.

Treatment of total urethritis and urethroprostatitis via the “Intramag” device, as compared to the traditional treatment methods, gives positive therapeutic results, and its application is recommended in complex treatment of patients of this group.

CHANGES OF ERYTHROCYTES RESISTANCE AND DEFORMABILITY IN ACUTE PANCREATITIS

Tbilisi State Medical University, Department of Biochemistry, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 79-81.

Membranes have been intensively studied because of their functional importance and involvement in many disease processes. It’s known, that certain diseases lead to alterations in the cell membrane. Thus, the investigation of the erythrocyte membrane in various diseases is of great importance. In given work attempts are being made to study physico-chemical properties of erythrocyte membranes, their osmoresistance and deformability in acute pancreatitis. According to our data, erythrocyte osmoresistance and deformability decreased in acute pancreatitis as compared to healthy persons. From our point of view, the cause of these changes is oxidative stress in inflammatory process, which leads to decomposition of the membrane.

THE ROLE OF NITRIC OXIDE IN PATHOGENESIS PERIODONTITES

INSTITUTE OF EXPERIMENTAL MORFOLOGY OF GEORGIAN ACADEMY SCIENCE

A.Natishvili Institute of Experimental Morfphology, Georgian AS;

Tbilisi State Medical University, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 89-94.

The aim of our study was investigation of nitric oxide content in oral cavit during different forms of periodontites.

It was established increasing of nitric oxide synthesis intensity with progress of disease/ That may be caused by activation of nNOS in salivary glands or cytokine induced intensification of expression of iNOS by macrophages and neitrophils. Elevation of nitric oxide generation on early studies of periodontites  protects gingival tissue from microbe damage. But with progression of disease disorders of mitochondria electron transport chain in the gingival tissue provokes development of ischemia, generation of reactive oxygen species and transformation of nitric oxide to the peroxinitrite, intensification free radical oxidation and destruction of cells. Nitric oxide decrease in gingival promotes development of insufficiency of antimicrobial defense mechanisms, disorders of blood circulation, tropic disturbances in tissues, elevation of platelet aggregation, lymphocyte proliferation, disorders cytokine balance    and contribute to development of inflammatory process and progression of disease. A cytokine mediator, nitric oxide involves in periodontal bon tissues remodulation, and pharmacological regulation of its synthesis and future metabolism may protect periodontal bon tissues from resobtion and degradation.

Structural indices of endothelium in the uterine leiomyoma nodes and intact myometrium

Department of Fundamental Medicine of State Medical Academy, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 94-94.

Background. Uterine leiomyoma is the most common solid tumors in adult women, affecting at least 30% during their reproductive years. Despite their prevalence, little attention has been directed toward the causation and pathogenesis of this disorder. Uterine fibroids rank as the major reason for hysterectomy in the world, accounting for approximately one-third of all hysterectomies

Materials and Methods: Humane myometrial and leiomyoma tissue was obtained from 20 women after hysterectomy. Tissue simples was investigated by light and electron microscopy procedure.

Results. Microscopically, the tumors were formed by interlacing bundles of spindle cells arranged in a whorl-like pattern. In the leiomyoma nodes the vascular network is less abundant. Endothelial  cells shows sings of minimal functional activity (luminal edge had no microvillus, vesicles are few and vacuole-like, basal lamina is degraded, nucleus are shrinkaged).

Conclusion.  Base on our findings we can conclude that endothelial dysfunction can play a pivot role in the pathogenesis of leiomyoma. Functional inactivity and  insufficient transendotelial transport activity lead tissue ischemia, which is a promoter of proliferation of the stromal cells.

COMPARING CHARACTERISTICS OF LIVER ISCHEMIC AND REPERFUSIONAL DAMAGES ON THE PHONE OF CHOLESTASIS IN EXPERIMENT

TSMU, Department of Topographic  Anatomy, Tbilisi, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 96-99.

The studies revealed that on the phone of 6-days long cholestasis after temporary switching the liver from blood circulation the alterative changes are characterized by specificity of intensity, arising time, duration and pathogenesis.

In the early stages of reperfusion in the liver there are present more severe changes due to necrosis of hepatocytes and increase of pressure in bile ducts on the phone of cholestasis besides  of ischemia of the liver.

THE IMMUNOBIOLOGYCAL ASPECTS OF THE OPERATIVE WOUND HEALING AND THEIR MEANING

Department of Surgery N4, Tbilisi State Medical University, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 99-101.

The Kavetski test has been applied on 199 patients with different types of the operative wounds(average age – 14-74 years) in order to detect the intensiti of the phagocytosis.Based on the peculiarity of the wound process flow, two groups of the patients have been distinguished. The first group(132 patients) included the patients with primary intention of the operative wounds, while the second group united 67 patients with complicated flow of the wound process. The percentage of the abovementioned groups by the principal disease, volume and type of the surgical intervention, type of the anesthesia, age, follow-up time and method of treatment were about the equal.

Our study showed that the phagocytic intensity factor has a positive feedback with the operative wound healing: the more high level of the phagocytosis coincides the less number of the frequency of complicated wound healing.

The association of HLA antigens with wound healing process was investigated in Georgian patients. Based on the peculiarity of the wound process flow, two groups of the patients have been distinguished. Control group was performed by 492 healthy persons.

Our study showed that the distribution of HLA antigens is different in the groups. The genetically determined risk of development of wound complication was significantly increased in individuals HLA-DR1, having the relative risk(RR) 3,56.

IMPORTANCE OF LEFT VENTRICLE MYOCARDIUM CONTRACTILITY ASSASSMENT FOR PATIENTS WITH CONGESTIVE HEART FAILURE

Tbilisi State Medical Academy, Chair in internal medicine, Georgia

J. Experimental and Clinical Medicine, 2006, 3, pp. 102-105.

Rationality: Congestive heart failure is one of the most prevalent complications of ischemic heart disease and appears to be the most frequent cause of mortality.

Nowadays, Ethiology and pathopiology hasn’t been studied completely. It is clear that contractility is difficult process and was parts (Systole and Dyastole) of this action are most important. The objective of this study was to evaluate the left ventricular stifness and to develop one more estimation method – how left ventrical will answer (without electrical activity during atrioventricular blockade in the experiment) to increased blood volume.

The study was conducted on dogs. Results showed, that during atrioventricular blockade (without electrical activity) increasing of enddiostolic pressure pushes left left ventricule to contruction.
