INFLUENCE OF INTRAVAGINALLY INSTILLED MISOPROSTOL  ON  SPONTANEOUS CONTRACTIONS OF MIOMETRIUM IN RABBITS

Objective: The assessment of the effect of misoprostol intravaginal installation on spontaneous contractions of the myometrium in rabbits.

Material and Methods: The experiment was conducted on 15 rabbits. After certain preparation, 200 mg of misoprostol suspension was instilled, and for on hour, spontaneous contractions of the myometrium were being recorded on a cymograph.

Results: The instillation of 5 microg/kg misoprostol stimulates in rabbits  strengthening and   prolongation of the myometrium spontaneous contractions. The peak of this effect is achieved at 22±4,3 minutes.

Conclusions: Misoprostol can be used prior abortion for prevention of hemorrhage and for reduction intra/postabortion bloodloss at 8 to 12 weeks of gestation.

Nutritional load as a new test PREDICTING EXACERBATION of Atherosclerosis in THE elderly

The aim of the research is to establish the influence of “Fatty breakfast” on the redox system, lipid spectrum, and fibrinogen in the elderly. There were investigated 155 IHD patients (80 females, 75 males), and 65 practically healthy persons – the control group (35 females, 30 males); average age was 69.5±5; IHD level was estimated by NYHA. We studied: lipid spectrum, fibrinogen, malondialdehyde (MDA) in erythrocyte membrane and in plasma, total and free bilirubin, erythrocyte osmotic fragility, and electrocardiographic parameters in IHD patients and practically healthy persons. The results are as follows: fatty breakfast causes dysbalance of the redox system and atherogenic lipids. Consequently, a disorder of micro/macro circulation, disregulation of myocardial metabolism and ion homeostasis, and electric instability of the myocardium develop. LDLC, fibrinogen, MDA in the erythrocyte membrane, and Q-T dispersion may be suggested as fatty breakfast determinant, which can reveal patients with high risk of IHD complication and persons with latent atherosclerosis.

ETIOLOGICAL STRUCTURE OF URETHRITIS IN MEN DETECTED BY MODERN TECHNOLOGIES OF MICROBIOLOGICAL RESEARCH

Microbiocenosis of urethritis is a permanently changing ecosystem. It depends on various factors, so in clinical microbiology is very important to control both quorum of causative agents and their sensitivity to new and old antimicrobial preparations. Along with theoretical purposes, that has a wide practical application.

The aim of this research was to study the etiology of urethritis in men, using high quality standard bacteriological technology – “BioMelieux France” (API and ATB test).

The study of materials taken from 40 patients with chronic recurrent disease showed that the dominant causative agent (in 31.7% of cases) is S.epidermis; the second place occupies S.aureus (17%); then follow: C. freundii, Corynebacterium spp, E.faecalis.

Monoinfections were determined in 76.2% of cases, and mixed infections - in 23.8%. S. epidermidis+E. faecalis was most frequently detected. Gram-positive causative agents dominated in 45%.

The Study of sensitivity spectrum showed that main causative agent – staphylococcus - revealed a high sensitivity to Gentamicine, Minocycline, Vancomycine, Teicoplanine, Rifampicine, Nintofurantoine; and resistance  against Norfloxacine, Levofloxacine, Oxacilline, Tetracycline and Penicilline.

ALTERATION OF SEVERAL PARAMETERS OF CELL ENERGOSUPPLEMENTARY SYSTEM IN HERPES VIRUS INFECTED PREGNANT WOMEN

The goal of the study is to investigate the relationship between changes of cell energosupplementary system and the development of phetoplacental insufficiency in pregnant women infected by herpes virus, suggested according to the positive anti-HSV-igG (high titers) and the increase of low avidity anti-HSV-igG. In 30% of women, there was revealed phetoplacental insufficiency and fetal intrauterine growth retardation, the decrease in lymphocyte citochrome-C, inadequately functioning ATP of mitochondrial breathing chain, decrease of NAD/NADH and NADP/NADPH redox potential, mitochondrial izocitratdegidrogenaze (ICDH), which is the aim of viral influence and leads to the cell damage. That’s why ICDH decrease may indicate a high risk of development of phetoplacental insufficiency.

DYNAMICS OF AVERAGE LIFE EXPECTANCY OF TBILISI POPULATION

From 1939 to 1986, life expectancy rate of the Tbilisi population increased by 17.9 years (54.1 to 72.0 years conformably). Over the following years, this index decreased to 69.2 years. By means of  the elimination charts it was determined that the reduction of cardiovascular pathologies could increase life expectancy by 7.5 years, the reduction of oncological pathologies – by 1.7 years, and the reduced number of trauma-related injuries could increase life expectancy rate by 1.5 years.

QUANTITATIVE CHARACTERISTICS OF STRUCTURAL ELEMENTS OF OLD RATS’ MYOCARDIUM AFTER  LONG-TERM PHYSICAL LOADING

The aim of the research is to study quantitatively the reaction of myocardial structural elements in the group of old animals under the lingering physical loading by swimming for  differentiation of morphological changes due to age load.

Material and methods. The experiments were performed on 20 male laboratory rats (mass 300.0 -450.0 g) aged 24-30 months. The physical load was modeled by swimming for 5 hours, 5 days a week. The material was taken on the 14-th day.

 

There were used the routine hystologic, electronomicroscopic, and histostereometric methods. Not trained animals of the same age (n=5) were used as a control group.

Results and their discussion. Under the physical load, against a background of age - related changes, the decrease of the volume of myofibrillar and vascular components and diminishing of the correlation between myofibrils and mitochondria were revealed. These changes defected the contractive function of the cardiomyocites.

The estimate of the quantitative and qualitative data of the ultrastructural shows that under the chronic load by swimming in old aged animals group is not adequate and may provokes the disturbance of the compensative processes.

EFFECTIVENESS OF AMIXIN IN TREATMENT OF GENITAL TRACT MIXT INFECTIONS

We have studied the effectiveness of treatment of genital tract infections by Amixin. 37 nonpregnant women with bacterio-viral infection were included in the main group. The diagnosis of viral infection was made by immunoferment method (detection of IgM and G) and bacterial - by cytological papanicolau smear. The control group consisted of 19 women with bacterio-viral infection who were treated by ethitropic medicines without amixin. The results of the study revealed that in the main group the treatment was much more effective having prolonged remission in the case of HSV, CMV infection, in all cases there was the improvement of clinical and immunological status. The currency of pregnancy in the main group (treated by amixin) was physiological.

USE OF MISOPROSTOL FOR  ABORTION

Misoprostol has been approved by the Food and Drug Administration (FDA) to be taken orally for the prevention and treatment of gastric ulcers associated with the use of nonsteroid anti-inflammatory drugs. It has also become an important drug in obstetrical and gynecologic practice. But a large body of evidence is needed to provide strong and consistent support for the use of misoprostol in this field. According to the data obtained from clinical trials, misoprostol is a part of most effective and safe regimen of medical abortion in the first trimester. The drug may also be used for cervical ripening, evacuation of the uterus in cases of embryonic or fetal death, and induction of labor, to treat postpartum hemorrhage. Although there are no published estimates of the extent to which misoprostol is currently used for obstetrical or gynecologic indications in Georgia, misoprostol has already become the drug of choice of many physicians for  the foregoing purposes.

STRUCTURAL REACTION OF NEWBORNS’ PERIPHERAL BLOOD CELLS AND PLACENTAL CAPILLARIES DURING MATERNAL CYTOMEGALOVIRUS INFECTION

Aim of the study. There were studied histo-/ultrastructural changes of the placenta and neonates’ peripheral blood cells under various activities of maternal cytomegalovirus (CMV) infection to determine prognosis and risk–factors of postnatal development.

Material and methods. The work is based on the findings obtained from 83 pregnant women and 25 newborns. The activity of CMV infection was studied using the ELISA method. Based on the received results, two groups were formed.

I group – 40 pregnant women and 14 newborns with active form of CMV infection (positive test on IgM and low avid antibodies of the IgG classes).

II group – 43 pregnant women and 11 newborns with reactivated form of CMV infection (positive test on IgM and high avid antibodies of IgG classes).

During different variants of CMV infection in the placenta, capillary wall permeability increases and chronic choriovilusitis develops.

The study shows correlation between the activity of maternal CMV infection and the degree of changes in newborns’ peripheral blood cells, which, associated with serological and immunofermental data, can be estimated as premorbid changes.

Single and long-term treatment with 0.5 mg\kg haloperidol suppressed formation of CRPA. 25 mg\kg of verapamil in single and long-term treatment has an antiamnesic action and partly restores amnesic action of haloperidol.

ROLE OF MINI-LAPAROTOMIC SURGERY IN TREATMENT OF ACUTE DESTRUCTIVE CHOLECYSTITIS

1002 operations have been performed using mini-laparotomic approach (females were 795, males -207, patients’ age varied 17 to 80 years). 278 operations were performed in case of destructive cholecystitis. 196 patients had phlegmonous and 82 patients – gangrenous form. The average duration of operation was 40-50 minutes. The average number of bed days was 2-3.

Cholecystectomies through mini-laparotomy may be realized for any form of cholelithiasis. This operation will allow reducing postoperative complications in high risk operation patients. The economic effect is 10 times better as compared to laparoscopy cholecystectomy and 5 times better than that of a traditional open laparotomy. This is the operation of choice in the treatment of acute destructive cholecystitis, with good postoperative and remote results.

Our aim is to define the degree of aggression and agitation in teenagers. For this aim, we have studied fifty schoolchildren using the questionnaire by Nemchin. The research showed that high index have both boys and girls. 32 percent of the researched are very emotional; most of them like to command. It’s necessary to draw more governmental attention to this problem, and find the ways for its solution.

INFLUENCE OF VERAPAMIL ON SIDE EFFECTS OF HALOPERIDOL

A combined therapy with 25 mg/kg verapamil and 3 mg/kg haloperidol determined that verapamil restores suppressed motor activity -  by 51.7 %, search activity by 58.9 %, and vertical activity by 29.7 %.  It was determined that in the combined use of 25 mg/kg verapamil and 0.5-3 mg/kg, verapamil prevents the prooxidant effect of haloperidol by decreasing peroxidation of lipids in different structures of the rat brain.

Development and formation of the most important functions in children are considered in the unity with maternal organism. Verious deviations from the normal course of pregnancy and labours charge adaptive processes of the fetus and newborn. In newborns of mothers having acute pyelonephritis during pregnancy, an increased level of β2  microglobulin is observed what testifies to some decrease of the filtration.

Morphological features of the placenta in pregnant woman with chronic pyelonephritis have been studied. There was revealed a considerable amount of sclerotic villi and necrosis reflecting compensatory - adaptive proceses.

ULTRASTRUCTURAL FEATURES OF BLOOD VESSELS IN MALIGNANT TUMOR OFHUMAN MAMMARY GLAND

The study of vessels has shown that initial stages of vessel wall formation depend on the accumulation of neutral glycoproteids in the endothelial cells.

The received results testify to the high contents of hyaluronic acid in the endothelial cells. The quantity of chondroitin sulfate C varies. The heterogeneity of metachromasia and different intensity of reaction with alcion blue and colloid by iron was observed in the endothelium and paravasal spaces, in the sites of tumor invasion and especially in the places of cancerous cells intrusion into the vessel lumina. The increase of NQP concentration and qualitative shifts in the AGG structure render certain influence on fermentative activity. And it expresses the activity of base phosphates - ATF and 5-nucleotidase - in the endothelial cells.

The main problem of social psychiatry is to provide maximal population research. The aim of the given work is to study the psychosocial condition of young people and to find ways for teen crime prevention.

We’ve researched two hundred students living in town and village. In Adjaria, statistics for under–aged criminal cases was as follows: 44 cases in 2004, 90 cases in 2005, 96 cases in 2006, a crime was committed repeatedly by 60%. It’s necessary to work out a governmental plan on the improvement of the adolescents’ psychosocial condition.

USE OF VERAPAMIL FOR PREVENTION OF EXTRAPYRAMIDAL COMPLICATIONS OF NEUROLEPTIC THERAPY

Single and long-term treatment with 0,5mg\kg dose of haloperidol caused extrapyramidal disturbances and suppressed motor activity in 51.9% of cases. Sulpiride suppressed motor activity in 49.1 % of cases.  25 mg\kg dose of verapamil has restored haloperidole - suppressed motor activity by 50%, and sulpiride - suppressed motor activity - by 48.3.

Religiosity exerts an antidepressive influence on the patients with arterial hypertension, which is proved by the existance of a positive correlation between the manifestation of psychological depression and figures of systolic/diastolic arterial hypertension. Hypertrophy of the left ventricle - a risk-factor of sudden death - is presented by low statistical indices in the believers with arterial hypertension. At the same time, in the group of unbelieving men is evident a high positive correlation of depression with the ejection fraction, with thickness of the left ventricle and carotid arteries. These facts confirm the data concerning the immediate connection of central hemodynamic lesions with the progression of arterial hypertension, with left ventricle remodeling that develops in parallel with the psychovegetative homeostasis.

USE PLAFERON LB FOR POSTCONDITIONING DURING EXPERIMENTAL MYOCARDIAL ISCHEMIA/REPERFUSION INJURY

In the acute stage of myocardial infarction (MI) focus of injury is formed by ischemia/reperfusion mechanism. Research goal was to study effect of Plaferon LB for postconditioning during experimental ischemia/reperfusion injury.

Materials and methods: 30 rabbits (2.5-3.0kg) were used in experiment. They were divided in 3 groups and 6 subgroups (5-5 animals). In I group experimental design of MI was performed by proximal ligation of left coronary artery (2-6hours). In II group on the 2 and 6 hour ligature was removed – reperfusion during 1 hour. In III group animals was treated by Plaferon LB.  The animals were under ECG monitoring. TnI was measured in blood.

Results: In II group after reperfusion TnI concentration was higher than in I group. In III group signs of injury was less evident than in reperfusion group.

Conclusion: Obtained data confirm effectiveness of Plaferon LB as postconditioning agent for prevention of reperfusion injury.

CONDITION OF DENSE FIBROUS CONNECTIVE TISSUE IN WOMEN WITH PROLAPSE OF SMALL PELVIS ORGANS: HISTOCHEMICAL AND MORPHOMETRIC ASPECTS

Pathogenesis of prolapse of the small pelvis organs in women can be connected with the anomalous changes of dense fibrous connective tissue composed of aponeurosis, fascia, and reticular layer of derma.

The aim of the study was histochemical and morphometric assessment of the aponeurosis of the oblique muscles of the anterior abdominal wall, round uterine and sacrouterine ligaments, and reticular layer of derma from the anterior abdominal wall and inguinofemoral region. The contingent consisting of 58 women was divided into 2 groups: I. the group of comparison - 22 women without clinical signs of internal organ prolapse; II. the main clinical group with isolated and combined clinical forms of  pelvic organ prolapse.

There was traced a reliable connection between the prolapse of  some organs of the small pelvis and dyselastosis, fibrosclerosis, sharp increase in histochemical content of GAG, suppression of  the specific potential of  the key ferments majority.

TREATMENT OF LIVER PRIMARY ECHINOCOCCUS BY HOMEOPATHIC METHOD

Treatment of liver echinococcosis has been provided by the homeopathic method. Parasitic cysts existing in the liver parenchima disappeared totally. Patients’ condition is stable now.
