CHANGES IN IMMUNE MECHANISMS IN THE PROCESS OF IMMUNOMODULATION DURING PERIODONTITIS

The content of CD3, CD4, CD8, CD19 lymphocytes and IgG, IgM, IgA in blood and the content of SIgA in saliva has been studied in the patients with periodontitis during the process of treatment with the antioxidant preparation phenovin and immunomodulator Una de Gato against the background of immunomodulation. It has been established that after treatment, the quantity of CD3, CD4, CD8, CD19 lymphocytes increases in blood and the ratio of immunomodulatory subpopulations is normalized. Despite the increase of CD19 lymphocyte quantity in blood serum, the level of IgG, IgM and IgA does not essentially change, while the level of SigA sharply increases in saliva. In the patients treated with a traditional scheme without immunomodulation, the changes in the above-stated parameters have a less expressed character.

Polycystic Ovary Syndrome –Variations of Clinical Classification  and Treatment Principles

Polycystic ovary is one the widest-spread reasons for the genesis of endocrine infertility. To receive the expected feedback from the treatment, a rational research was done concerning the primary pathogenesis of polycystic ovary. The choice of an optimal therapy is of high importance both for clinicians and practitioner doctors. Hence, the aim of the research was to classify polycystoses according to their pathogenesis and to select a research design and treatment principles.

105 patients (aged 16-38) with polycystic ovary syndrome were studied. The patients underwent objective, general clinical, in some cases - biochemical, hormonal and instrumental researches; 4 groups were singled out after the research: polycystic ovary developed as a result of Hypothalamic  syndrome, metabolic syndrome, adrenogenital syndrome and Hyperprolactemia syndrome. In accordance with the research, totally different therapies were prescribed to these groups with the aim to correct the main disease and to recover the reproduction and regenerative functions of the ovary. As the research has shown, surgical operation was necessary only in 29 cases, which again proves that conservative correction is far more effective for this type of pathology.

Actual aspects of treatment for arterial hypertension

in patients with psychic disturbances

It is known that psychic disturbances which are rather frequent in patients with arterial hypertension affect negatively and complicate the clinical picture of the disease. The aim of our research was to study the problem of the treatment of arterial hypertension in patients with psychic disorders and clarify clinical activity and safety of combined therapy with angiotensin II antagonist eprosartan and antidepressant paroxetine.

67 patients with arterial hypertension and psychic disorders received eprosartan (600mg daily) with 20 mg of paroxetine for 16 weeks. Experimental data demonstrated that combined therapy effectually controlled blood pressure (the data of 24-hour blood pressure monitoring detected antihypertensive effects in 67.8% of the patients). Moreover, the antihypertensive effect was accompanied by the improvement of clinical psychiatric parameters and standardized questionnaire data (decrease of anxiety and depression according to the Hamilton Rating Scale by more than 50% and the improvement of some parameters of quality of life). Thus, the research results suggested clinical activity and safety of combined therapy with angiotensin II antagonist eprosartan and antidepressant paroxetine in patients with arterial hypertension and various psychic disturbances (anxiety and depression).

Investigations of the oral cavities of 685 refugees living in Azerbaijan and 680 persons of the control region – Agdjabedi have revealed that the incidence of caries and gingivitis among the refugees equaled 30.0±1.75, but this indices among the Agdjabedi population were 23.8±1.63 (t=2.6). The same ratio was revealed in case of combined caries, periodontitis and parodontosis. Thus, the refugees have a higher level of caries in comparison with the people of the control group: a higher predisposition to caries was revealed among the refugees.

THE INFLUENCE OF ARTERIAL HYPERTENSION ON THE PARAMETERS OF CLINICAL-METABOLIC CONDITION OF MALE PATIENTS WITH TYPE 2 DIABETES

The purpose of this work was to research a correlation between HbA1c and other risk factors of vascular complications of diabetes in men with type 2 diabetes in the presence or absence of arterial hypertension. A representative group of 384 men with type 2 diabetes was observed.

In the absence of arterial hypertension, HbA1c correlated with the age of the patients (r=+0.21±0.101; P<0.05), with diabetes duration (r=+0.40±0.095; P<0.001), patients’ weight (r=-0.28±0.099; P<0.01), BMI (r=-0.25±0.100; P<0.05), SBP (r=+0.23±0.100; P<0.05), and fasting glycemia (r= +0.24± 0.100; P<0.05).

In the patients with arterial hypertension, HbA1c correlated with fasting glycemia (r=+0.27±0.091; P<0.01), VLD cholesterol (r=+0.42±0.105; P<0.001), Triglycerides (r=+0.19±0.094; P<0.05).

PROGNOSTIC IMPORTANCE OF BRAIN CORTEX BIOELECTRIC ACTIVITY STUDY IN CRITICAL CONDITION

26 patients with acute pathologies were observed. Electroencephalograms were done on the Hungarian 16-channeled bipolar encephalograph “Medicor“ EEG-16. We also used light and audio irritation.

The observation displayed that in the terminal state of an organism, caused by different etiological factors, bioelectric activity of the brain sharply changes. In most patients the following changes were marked: the absence of the main rhythm, domination of a slow pathological polymorphous activity “Plane EEG”, also the “electric silence” zone, and as a rule – no response to the peripheral irritations.

From these facts we can conclude that in critical states, sharp disturbances of the brain take place. The electroencephalographic method allows functional assessment of critical state patients and can be helpful in diagnosing the origin of coma to predict the outcome, and for the monitoring purpose.

EXTENDED LYMPHODISSECTION FOR GASTRIC CANCER AND “STAGE MIGRATION” PHENOMENON: CAN IMMUNOHISTOCHEMICAL EXAMINATION DETERMINE THE STAGE MORE ACCURATELY?

It has been shown that N-category gastric cancer can not be identified accurately in 48.1% (50/104) unless extended lymphodissection is performed (p<0.001). Taking into account the fact that immunohistochemical examination can find metastases in 22.2% of non-metastatic lymph nodes (p<0.01), assessed by pathohistologic techniques, it is reasonable to think that the former method was able to transform the N1 category to N2 in 8 patients. Therefore, if we summarize the received data, we can see that an incorrect assessment of N-stage gastric cancer occurs in 55.8% (58/104, p<0.001). Summarizing our data and calculations, it’s possible to say with certainty that approximately in every 3 of 5 patients who underwent standard resection (D1 lymphodissection) the N stage of gastric cancer is evaluated inaccurately.

Combined specialised stomatologic and prospective inspection of the adult population of Baku has shown that periostitis, now prevailing among the adult population, has acquired great medical-social importance.

A social, epidemiological survey of periostitis found out and important elements composing its parameters: seasonal and social-profession dependence of the illness, general and mouth cavity hygiene, attendance of health care institution , hygienic behaviour, motivation etc. Based on the objective data, we have worked out an effective system for the prophylaxis of periostitis.

The state of the cochleo-vestibular function and muscular bioelectrical performance have been studied during varied cerebral disturbances, particularly during discirculatory pathologies of the cerebral vessels, Parkinson diseases and extensive process of the posterior cranial fossa.

Anamneses of all the patients have been gathered and they consulted an otolaryngologist, neurologist and other specialists.

Various tests – audiometry, vestibuleometry, electromyography, radiography of the brain (head), Doppler ultrasound scan of the cephalic and cervical vessels have been performed.

On the basis of the conducted study, it can be noted that in the above-mentioned abnormal process, functional state of the vestibular system leads to the changes in muscular tone. Although no relation has been observed between bioelectrical performance of the muscles and nystagmic direction.

Based on the study of the above-mentioned issues, we can stress the diagnostic value of electronystagmography and electromyography. In this case, the reliability of the obtained data simultaneously increases regarding the findings of the clinical survey and other diagnostic methods of investigation.

Thus, the analysis of the carried out research allows us to draw a conclusion that during magnetotherapy of inflammatory diseases of the oral cavity mucous membrane, the optimum mode of magnetic field batching is 20 mTl inductivity and that of the exposure - 10-15 minutes. 15-20 sessions are recommended in the course of treatment, depending on the degree of severity of an inflammatory process.

A social-epidemiological analysis of helminthism first of all shows its social dependency and susceptibility to epidemiological patterns. Lots of social-epidemiological factors, both favourable to the spread of helminthism and limiting its spread among children, open wide prospects for organization and realisation of preventive measures against massive contamination of the population of children.

Undoubtedly, a due parasitological inspection, differentiation of forms of the agents causing helminthism and carrying out an effective, specific treatment still remain effective measures both for preservation of children’s health, and for the decrease of the disease spread. However, having revealed social-epidemiological connection of helminthism, authentically having estimated the factors favourable to or limiting its spread, we believe in a huge importance of the realization of preventive measures directed to the decreasing the morbidity among children.

PATHOGENETICAL AND IMMUNOMODULATORY EFFECTS OF VIFERON ON THE CYTOKINE SYSTEM OF CHILDREN WITH ATOPIC DERMATITIS

The purpose of the study is to determine IL-lα, IL-4, IFNg before and after the treatment with viferon - a medical preparation of interferon for the treatment of atopic dermatitis and similar diseases. There were examined 74 patients with atopic dermatitis and 27 practically healthy children at Baku Clinical Children’s Hospital No 6. As a result of this medical examination, it was revealed that in the patients with a mild clinical course IL-lα - 117.7±7.0 pq/ml (p<0.001), IL-4 - 91.5±6.2 pq/ml (p<0.001), IFNg - 51.7±3.1 pq/ml (p<0.001), and in the patients with a moderate clinical course IL-lα - 208.14 ± 12.6 pq/ml (p<0.001), IL-4 - 138±8.8 pq/ml (p<0.001), IFNg - 31.12±2.1 pq/ml (p<0.001), in the patients with aggravated clinical course IL-lα - 253.05±14.8 pq/ml (p<0.001), IL-4 - 166±0.7  pq/ml (p<0.001), IFNg- 17.3±0.9 pq/ml (p<0.001).

The indices of the healthy group were: IL-lα 37.3±0.4 pq/ml, IL-4 47.8±0.9 pq/ml, IFNg 80.7±0.9 pq/ml. As the indices of the patients with the mild clinical course are near to those of the patients of the healthy group, there will be no need in the correction with the medical preparation viferon. After receiving the medical preparation viferon, in the patients with moderate clinical course IL-lα has decreased by 2.9 and in the patients with severe clinical course – by 2.4 times, the level of IL-4 reduced in the patients with the moderate-severe clinical course by 2.4 times, and in the patients with severe clinical course – by 2.2 times; the level of IFNg increased in the moderate-severe clinical course by 2.3, and in the patients with severe clinical course by 3.7 times.

In the pathogenesis of atopic dermatitis IL-lα, IL-4, IFNg play key part and are changing depending on the severity of the disease.

AGE-RELATED PECULIARITIES OF MICROCIRCULATION DISORDERS IN MIOCARDIUM DURING ALLOXAN-INDUCED DIABETES

The experiment results show that microcirculation disorders similar to those caused by alloxan in old diabetic patients take place in the cardiac muscles of domestic rabbits.

Under normal conditions, in the cardiac muscles of old patients the number of capillaries is decreased. In old animals’ heart muscle capillaries are shortened and widened in many cases too.

In the left ventricle blood vessels of diabetic animals, alloxan-induced erythrocyte aggregation is marked. But no microcirculation disorders are noticed in young animals’ cardiac muscles, whereas in middle-aged animals the left ventricle capillaries are in disorder.

In old animals’ blood vessels we found aggregations not only in the left ventricle, but in the right ventricle too.

Thus, we came to the conclusion that we deal with the development of micronecrosis which causes cardiac insufficiency in elderly diabetic patients.

Purulent-septic diseases remain a serious problem, especially in obstetrical establishments in which newborns because of a high susceptibility and weakening of mother’s organism are subjected to pyoinflammatory diseases the level of which changes from 5 up to 15% in newborns and from 4.6 up to 11.9 % in parturients.

Researches are carried out in the Baku S.Aleskerov maternity hospital ¹5. There were taken tests of air, washouts from the objects in the obstetrical hospital. A bacteriological research of the air was performed in six departments of the maternity hospital. In total, 128 samples of air were investigated within the period of 2004-2007.

For the first time the scientific experimental research of an obstetrical establishment necessary for microbiological monitoring of the spread of nozocomial cultures has been carried out in the republic. The Results of the experimental researches allowed us to establish the etiologic structure and biological properties of conditionally pathogenic microorganisms in the obstetrical establishment. The expediency of application of a set of differentially-diagnostic, selective nutrient mediums and microtest systems for indication and identification of conditionally pathogenic microorganisms was experimentally proved during bacteriological diagnosis of purulent-septic diseases.

The article reports on the research that has been carried out to study the primary risk factors for hearing impairment in Azerbaijanian children for the purpose of providing auditory screening on the basis of the given risk factors. 1003 newborns to 2 years were examined by the transient evoked otoacoustic emission (TEOAE) method and the risk factors of their hearing loss were investigated. As a result of auditory screening among 44 babies, SNHL was diagnosed. The frequency of risk factors occurrence was compared in normal babies and those with hearing loss. As a result of the research, the primary risk factors of hearing loss in Azerbaijan proved to be marriage between close relatives, SNHL in the family members and close relatives, usage of aminoglycosides (gentamicin, kanamycin), weight of newborns lower than 2000 gm, birth trauma and asphyxia.

CHANGES IN BONE DENSITY AND THE MAIN INDEXES OF BONE 

METABOLISM IN JUVENILE ARTHRITIS

The investigation aim was to study the density of bone tissue, bone metabolism and the main systemic hormones regulating bone metabolism in juvenile arthritis.

The results of the performed investigation have exposed the decrease in the density of bone tissue. Mineral composition and mineral density of the lumbar vertebrae (JCA-JRA) decreased down to 37.0% -18.0% and 50.0-24.0%. A correlation was revealed between the illness duration and density of vertebral bones in patients with JRA. A resorption increased and lingering bone tissue formation was established. The content of oxyprolin in the blood serum (JCA-JRA) increased 1.5-1.6 times, content of osteocalcin decreased 1.3-1.4 times. In both forms of juvenile arthritis, the amount of total calcium and inorganic phosphorus in the blood is low (p<0.001-0.05). The amount of magnesium in the patients with JCA increased (p<0.05), in the patients with JRA - decreased (p<0.001). A positive correlation was revealed between the illness duration and calcium amount in the blood of the patients with JRA. It was also revealed the impairment of secretion of the main systemic hormones regulating bone metabolism: content of parathormone in the blood (JCA-JRA) increased 2.0-2.7 times, content of calcitonin increased 3.0-2.5 times.

The results of the performed investigation exposed changes of bone density, bone metabolism and the main systemic hormones, regulating bone metabolism in both forms of juvenile arthritis.

Features of THE cOURSE OF pregnancy IN women with diabetes

The researches have shown that the course of diabetes during pregnancy proceeds in a wavy mode, with propensity to ketoacidosis, hyper and hypoglycaemic conditions.

In the first weeks of pregnancy, diabetes in the majority of pregnant women remains without changes, or shows some improvement of tolerance to carbohydrates, which stimulates producing of insulin by the pancreas. In the second half of pregnancy, due to the raised activity of hormones (glucagons, placenta lactogen, prolactin) the tolerance to carbohydrates impairs, diabetic complaints amplify. The third stage is connected with the delivery and postnatal period. During delivery there is a danger of metabolic acidosis which can quickly proceed into diabetic one. During lactation the need for glucose is lower than before pregnancy, which doubtlessly tells on the change of hormonal balance conditioned by pregnancy development.

EVALUATION OF NON-LOCALISED FORMS OF PROSTATIC ADENOCARCINOMA ACCORDING TO THE GLISSON RATING SYSTEM

Evaluation of the Glisson Rating System for non-localised forms of prostatic adenocarcinoma showed that the Glisson values were the same in the biopsy samples as in the operative material from 62 patients (81.6%). The value of 1-5 scores was detected in 23 cases (30.3%) of biopsy samples and in 11 patients’ (14.5%) postoperative material. The value of 8-10 scores was detected in 22 cases (28.9%) of biopsy samples and in 30 patients’ (39.5%) postoperative material.

PLASTY OF GINGIVAL PERIIMPLANT FOR SINGLE TOOTH IMPLANTATION

Introduction. Tooth loss as a result of extraction, traumas or parodontal diseases cause  damage to the anatomy, function and aesthetics of the dentoalveolar systems, influencing oral rehabilitation and achievement of effective results. A considerable interest attracts the development of high effective techniques of surgical procedures directed to the restoration of natural morphology of soft tissue surrounding an implant.

The purpose of the study is the increase of aesthetic results of oral rehabilitation during dental implantation in aesthetically important zones.

Materials and methods. Five patients with single defects of the dental ridge in the maxillary frontal site were presented in this study: 2 with the absence of a lateral incisor, 3 - with the absent central incisor. Dental implants were inserted in the alveolar ridge defects of these patients and a crown was fabricated. There were used implants by Hi-Tec (Israel) – 3.75mm in diameter and 16mm in length. The next stage consisted of surgical formation of the gingival periimplant. This procedure pursued the aim of increasing the aesthetic effect of the prosthesis.

Research results. This study has shown a high aesthetic effect in the crown site after the surgical procedure on the papillary and marginal gingiva.

Conclusion. This technique is recommended for achieving a higher aesthetic effect in aesthetically important zones.

The problem of intrauterine infection is one of the most important problems of contemporary medicine. Severe consequences of intrauterine infection and especially damages to the central nervous system of children impart a huge medical-social importance to the problem. In this regard, chlamydia infection becomes an increasingly meaningful among the pathologies threatening newborns and infants. The analysis of complex investigations indicated that chlamydia infection has a negative influence on the process of gestation, delivery, and on the health of newborns as well. Subclinical development usual for this disease promotes a long lasted persistency of the germ, which requires purposeful investigation of the babies born to the mothers with chlamydiosis. Chlamydia infection of the CNS plays a leading role among perinatal damages. Newborns without identified chlamydia were indicated by primarily latent, but more severe damages with a tendency towards increase, causing a delay in psychomotor development.

The research goal was to determine a possible role of hereditary factors in the genesis of post-term pregnancy. The following methods were applied: population-epidemiological, clinical-genealogical, genetical-correlative analysis, segregation analysis. There were established: population end family frequences of post-term pregnancy, index of phenotypical correlation and index of heredity. 22 risk factors were defined. The index of segregation frequency was established as well.

The results of the performed researches have shown that nowadays intestinal parasites (ascarids, lamblia) are widely spread among the children in our republic. Besides, in the last years, due to the social-economical and ecological changes in the republic, the interrelation of parasites have changed, influencing clinical therapy and its results. At the same time, dysbacteriosis is observed among the children with intestinal parasites. Clinical conditions and chemical therapy of dysbacteriosis must be taken into account when treating intestinal parasites.

The role of IL-8 –251T/Apolymorphism in THE pathogenesis of endometriosis

This study investigates whether IL-8–251 T/A gene polymorphism could be used as markers of susceptibility in endometriosis.

50 clinically healthy women and 70 women with endometriosis were under our control. We applied physical, instrumental, genetic studies.

 IL-8 – 251 T/A related genotype and alleles increase the susceptibility to endometriosis.
