The paper contains an analysis of activities of the Clinical Department (Sector) of the Tbilisi State Medical University directed to optimization of clinical education of students. The main difficulties relate to the necessity of successful combination of clinical, scientific and pedagogical activities of teachers, relations with the authorities of non University private clinics, lack of financial resources for promotion of mobility of students and teachers, invitation of competent scholars from abroad, due equipment of virtual clinic (the center of simulation clinical education). Overcoming of the above-mentioned challenges can essentially contribute to the further improvement of medical education.

CNS structures such as almond-shapes structure, hippocampus, prefrontal cortex influence on the decrease of memory based on negative emotions that was more shown in less aggressive animals, than is non-aggressive. We can conclude that the difference in both aggressive animals is very little than in non aggressive. That suggests the high quality of memory and consolidation of aggressive animal.

Thus we can conclude that an aggressive rat, whether it has more or less anxiety, tries to perceive the environment and select safe condition that is useful to it, what is contributed by features that are characterized for aggressiveness which is manifested by working capacity.

Also in open field we have seen that both aggressive animals are characterized by a good ability to perceive the environment, the duration of grooming, that helps them to make a correct decision, also set of search elements that are characterized only for them. Exactly these are most important features that are characterized for aggressive animals and that they have since birth and the development of social hierarchy has no significance.

Demand on prebiotics increases on the global pharmaceutical market. Compared to chemotherapy drugs they aren’t potent drugs, are spread in food products, aren’t toxic and don’t have side effects and the most important – have no immunosuppressive effect.

Prebiotics have unique mechanism of activity; non-pathogenic bifid and lacto bacteria of colon are a target organ; they promote their growth and increase, which ensures activation and resistance of immune system.

Prebiotics have anti-cancer effect together with other useful effects. So, it added motivation to usage of prebiotics in medicine.

Search and screening of natural resources with prebiotic activity is carried out throughout the world.

New prebiotic Rumofosi is an intellectual property of Larki LTD, trade mark and manufacturing technology are protected. Rumofosi is made from hydrolysis of endemic corn materials, which are spread in mountaneous areas of Georgia. Standardization of Rumifosi, pre-clinical and experimental research on animals for evaluation of efficiency and safety of Rumifosi is carried out at the present moment.

THE ASSESSMENT OF THE QUALITY OF MEDICAL CARE PROVIDED TO PATIENTS WITH CHRONIC GASTRITIS AND PEPTIC ULCER DISEASE

The article is devoted to the assessment of the quality of medical care provided to  patients with most common diseases of digestive system in outpatient and hospital phase.

It was found out that in 66.3% cases of patients medical care provision, there has been registered a discrepancy of provided medical care quality, 43.4% from which had negative effects on patients' health status. The greater part of inconsistencies was due to the inappropriate way of gathering information about the disease (49,0%).

It is getting more and more necessary to use regularly the automated technology method of healthcare quality peer review.

Saliva can be considered as a biological fluid, composition and properties of which contributes to the maintenance of the mineral components and homeostasis of the oral cavity. Based on the literature and our data, we can say that when planning the prophylactic treatment of the adolescents, there must be considered the changes of saliva components and properties, caused by different somatic changes. In adolescents’ thyroid dysfunction (hypothyroidism) there is a high degree of susceptibility for caries and poor quality mineralization of saliva potential, which leads to the 3rd type saliva crystallization, while in the control group there is a high-quality mineralization of saliva potential and type 1 saliva crystallization.

The Study of the Impact of Apiculture Products on Some

Physiological Characteristics of the Central Nervous System

The influence of non-alcoholic beverages, which are made using honey, pollen, hive-dross and propolis, on the changes in activity of anxiety behaviour has been studied. 

The prophylactic efficacy of drinks has been established. It is shown that their use can significantly improve the functional characteristics of the central nervous system, increase the psycho-emotional state and the power of memory of the experimental animals and reduce anxiety-like behavior.

The obtained results indicate the appropriateness of the use of these drinks for the prevention of neurotic diseases.

Preparation Rumifos represents sum of biologically active substances. It’s obtained by means of cereal hydrolysis of endemic cultures spread in Caucasus. Manufacturing technology and trade name of Rumifos is a private property of ,,Larki” and all rights are reserved.

Because of specificity of the preparation it represents multi-componential heterogeneous system, which emerged us to develop different methods of its standardization. We determined dominating substance content in the preparation, that were: Inverted sugar (not less than 0.15%); Phenols (not less than 0.15%); Total quantity of organic nitrogen (not less than 0.03%); And we developed the standard for checking quality of 10% solution of Rumifos.

PROXIMAL SODIUM REABSORPTION AND CIRCADIAN RHYTHM OF BLOOD PRESSURE IN SALT-SENSITIVE HYPERTENSION

Proximal sodium reabsorption is considered to be the key determinant of response of blood pressure to sodium intake. Salt sensitivity and circadian rhythm of blood pressure might the keys to understanding the connections between cardiovascular and renal complications. Based on aforementioned, the overall goal of our study is to elucidate the role of proximal sodium reabsorption and circadian rhythm of blood pressure in the development of salt-sensitivity and salt-sensitive hypertension. It was examine 140 subjects – 70 normotensives and 70 hypertensives (JNC 7 stage I). All of them was typing by salt-sensitivity test. Also we assess proximal sodium reabsorption and circadian rhythm of blood pressure.

The results of this study clarify interrelationship between main causative factors leading to salt-sensitivity: inadequate proximal sodium reabsorption and specificities of blood pressure circadian rhythm.

DEPRESSION  AND  CANCER

The study included 100 patients from 25 to 75 years. Out of them women were 60, Men - 40, surgery had only 45 patients, chemotherapy – 1, after radiation therapy - 4, combined therapy - 35 patients. 6 were suffering from lung cancer, 22 - breast cancer, 6 - skin cancer, 7 - gastric cancer, prostate and  bladder cancer - 4 patients, 15 - ovarian cancer, uterus cancer – 25. 
From 100 patients 20% was suffering from mild depression, the average - 44%, severe - 35%, without depression - 1%.  Suicidal taught was detected in 45% of surveyed patients, a panic or somatic-vegetatic disorders - 79% (headache, dizziness, palpitations, and weakness). A large part of the patients, 80% complained of sleep disturbance (difficulty in sleeping, in the middle of the night awakening, early awakening), some of the  symptoms of asthenia was found in 56%. 

Psychological rehabilitation centers for cancer patients is necessary to establish on a base of oncology clinics. Professional psychological help will improve oncology patients “quality of life”. Cancer patients diagnosed with major depression may benefit from a combined modality approach that includes both psychosocial and pharmacologic interventions. Psychosocial treatment approaches that may be of value include those that provide information and support and those that address any combination of emotional, cognitive, and behavioural factors.

Since first publications about metabolic syndrome large amount of data about its clinical significance appeared. The series of investigations have demonstrated the important role of this syndrome in development of diabetes mellitus, coronary heart disease, atherosclerosis and arterial hypertension, often accompanied by dangerous complications. Nevertheless, controversial points now exist regarding diagnostic criteria, which allow us to identify metabolic syndrome in scientific studies and routine practice. In this review we made an attempt to analyze different approaches to this problem in light of results of contemporary investigations.

Common reasons of premenopausal bleeding and their early manifestation

In premenopausal women anomal uterine bleeding can be the cause of hormonal disbalance,  but also a premise of malignant desaese. We study 153 premenopausal aged-woman personal medical records during 2012-2013 retrospectively in our hospital with complaints of  bleeding. This records containing histomorphlogically determined nozological units which were mainly bleeding causes. We established that for prevention of malignant deseases it is important to perform  histopathological examination yet in premenopausal age.

FORMATION OF SPATIAL REPRESENTATION OF AGGRESSIVE AND

Aggressive animals are characterized by higher reproductive capacity for memorization in open space. High rate of aggressiveness help them in this. In this particular case aggressive animals have better navigation learning than non aggressive ones, formation of spatial representation of platform location develops through various orientations.  This kind of mental map could be perceived by the rat or not. In this case, this chart is well perceived by both aggressive animals.

At present number of patients, who are in need of surgery assistance is being steadily increased together with the number of the combined emergency intervention. Purpose of our work is analyzing 254 simultaneous operations conducted on the basis of the Academician Ghudushauri National Medical Centre and the Institute of Obstetrics, Gynecology and Perinatology named after Chachava. We have allocated clinical pairs, which are met the most often: Gynecological component – hernioplasty; Gynecological component – cholecystectomy; Gynecological component – appendectomy; Caesarean section – hernioplasty.

Leptin was assessed to play a coordinating role in obesity and its cardio-vascular complications, however the findings are conflicting and further clinical investigation is required. The aim of the present study was to evaluate the association of serum leptin with cardio-vascular risk factors in different body mass index and age groups. One hundred and forty nine female patients were enrolled in the study and divided into groups according to body mass index (BMI) and age. Following measurements were carried out: height, weight, BMI , waist circumference, blood pressure. Venus blood sample was obtained for plasma leptin, insulin, glucose and lipid profile analysis. Insulin resistance index was calculated for each patient. Body fat distribution was measured using Dual energy X-ray Absorbtiometry. The lowest leptin concentration was observed in overweight patients, the highest concentration was seen in obese patients. The difference between leptin levels were not observed in age groups. Leptin positively correlated with high density lipoprotein cholesterol levels in obese and elder patients. Leptin might act as a preventive measure for cardiovascular complications only in the presence of sufficient amount of fat mass. Further studies are warranted in order to support these results.

Screening Study of Iodine Deficiency Status and Thyroid Diseases in the republic of Georgian Children

The objective of the study is the screening of thyroid diseases among school age children in Georgia as an iodine deficiency region.

Materials and Methods: 52 328 children with age range 6-16 years were included in the study. Children were screened by palpation and ultrasonography of the thyroid gland, after that, with children who have had a change were made: TSH and Anti-TPO tests. According to the laboratory and clinical conditions we divided children into five groups: 1) without changes, 2) with hypothyroidism, 3) with hyperthyroidism, 4) with nodular goitre, 5) with autoimmune thyroiditis.

Results: The children were distributed in the following way: 1) without changes endemic goiter: 4 403, 2) with hypothyroidism 303, 3) with hyperthyroidism 18, 4) with nodular goitre 27 and 5) with autoimmune thyroiditis 675. The appropriate treatment according to the laboratory and clinical conditions was prescribed to all patients.

Conclusion: Thyroid gland diseases represent an important medical and epidemiologic entity, as its deleterious effects on patients is strongly recognised. As our study demonstrated thyroid gland diseases in Georgian children segment is one of the major problem. A supplementation program was conducted by government in purpose to improve the iodine-deficiency status, but it is not enough to stop IDD.

Androgen deficiency and insulin resistance in obese male patient

Background: The relationship between obesity and testosterone levels is one of the longest running controversies in endocrinology. The objective of the study is to show correlation of obesity and insulin resistance and testosterone levels in male patients. We also studied the influence of testosterone replacement therapy on obesity and insulin resistance in men.

Materials and Methods: 107 subjects with BMI 27,0 – 48,0 kg/m2 were enrolled in the study. The following analyses were done: anthropometric study, biochemical measurements and ultrasonography. According to the clinical condition we divided patients into three groups. The appropriate treatment was prescribed to all patients.

Results: In all investigated patients abnormal lipid profile and increased level of leptin was observed, all patients had decreased level of free testosterone and had inversely correlated with the degree of obesity, but not insulin resistance. After three months of treatment: Free testosterone level increased in all groups but the best results was in III group which was treated by diet, physical activity, metformin and testosterone. HOMA–IR decreased in all group but I and III group had alike result. BMI decreased in all groups but bets results was in III group. Leptin level after treatment was approximately same in all groups,  but compared best  results was achieved in III group.

Conclusion: Testosterone has the leading role in the etiology of obesity.As our small study demonstrated that it is possible to break into this vicious circle by raising testosterone levels in obese men with insulin resistance and low testosterone level.

SIMULTANEOUS OPERATIONS OF COMBINED CHOLECYSTECTOMY IN GYNECOLOGY

Gallstone disease is one of the most distributed surgical disease and the reason for great number of hospitalization of the patients. Expression of cholecystitis and combined gynecologic pathologies is being increased reaching 20-25% annually due to the developing diagnostic facilities. Our Clinic has conducted 34 Cholecystectomies in combination with the gynecological operation components. The Patients researched by us were divided into three groups, differing in the methods of operational approaches.

Laparoscopic cholecystectomy was conducted in combination with gynecology laparoscopy in the first group. In the second group – laparoscopic cholecystectomy in combination with laparatomical gynecology and the third group included the patients who underwent both stages of operations with laparatomic methods of approach.

Simultaneous operations, in particular, such widely developed operations in surgical practice as cholecystectomy and gynecologic operations by using laparoscopy devises, have number of indisputable priorities.

Postpartum acute infections’ etiologic factors and their  clinical course of the peculiarity

Retrospective research was conducted at V.Bochorishvili Sepsis Center, Gynecology department, on  clinical records of postpartum patients with complications due to infection. 310 clinical records dated from 2006 to 2012 were studied. During the research the focus was on pathologies caused by acute infection during postpartum period, etiological factors, peculiarities of their clinical development, contributory reasons, age. The second stage of the research was conducted at the basis of “LTD Davit Gagua Clinic”, women’s consultation centre, vaginal flora of 200 healthy pregnant women was studied by bacteriological method to determine the role of conditionally pathogenic flora. By retrospective and prospective research different clinical manifestations of infectious pathologies were determined and identified at different etiological factors, risk-factors, age groups. It must also be marked that during the study of infectious etiological spectrum of postpartum patients some kind of approximation to percentage of agents, which were got in healthy pregnant women, according to bacteriological study result of vaginal flora, which might represent and increase the risk of acute infections while using invasive methods in the upper genital tract during the delivery process.

Placental angiogenesis in norm and pathology (experimental preeclampsia)

Hypoxia of utero-placental complex is one of important pathogenetic link of preeclampsia
(PE), accompanied by damage of blood vessels endothelium and release of several vasoactive compounds that violate vascular tone and microcirculation in the mother's body, that leads to decrease in perfussion of vital organs and the placenta.

The aim of the study was to establish a morphological alterations in placenta during experimental preeclampsia.

For the purpose o fmodeling of preeclampsia in pregnant rats, at 10-th day of gestation the lumen of the abdominal aorta below the renal artery was narrowed by the silk threada third of its diameter (0.2 mm).

Results: during preeklampsii at the end of the second trimester (18-day,s hort-term hypoxia) in the placenta of rats revealed common acute dyscirculatory changes with pronounced dilatation of the lumen of blood vessels, stasis and hemorrhages; in most cases detected (71%) the compensatory-adaptive transformation with fetal activation of angiogenesis were detected, however in 29% - dystrophic changes with a slight angiogenesis. At the end of preeclampsia in the third trimester (24 day, long-term hypoxia) in the placenta of rats, an in fringement of the process of placentation, involutional-destructively change in varying degrees, a violation of fetal angiogenesis revealed. Among them, in 37% of cases the changes were relatively mild, and 63% - identified heavily expressed changes incompatible with life. Involution-destructively changes and compensatory transformation in the placenta in rats correlates with the duration of preeclampsia and severity of the pathological process.

CHEMICAL CONTENT AND PHARMACOLOGICAL ACTIVE COMPOUNDS FROM THE ROOTS OF CEPHALARIA GIGANTEA GROWING IN GEORGIA

Cephalaria gigantea is an endemic plant of Caucasus growing in Georgia. The roots of this plant contain a large amount of triterpene saponins, alkaloids, phenolic compounds and are well-known in traditional medicine as sedative and anti-inflammatory remedies.

The present paper describes the chemical content and pharmacological activities of crude extracts, triterpene glycosides and alkaloids. Antifungal, antiprotozoal, anti-inflammatory, antiplazmodial, cytotoxic and anticonvulsive activities of the extracts and individual compounds were investigated in vitro experiments.

The study has shown that structural-functional changes in microadenoma were similar to those which developed in adenoma. Thus, we supposed that microadenoma and adenoma are the different stages of  one  process.
