PENSION PROVISION ISSUES OF THE ELDERLY POPULATION OF GEORGIA

The article refers to the implementation of the pension system reform in Georgia, which on the one hand envisages reforms carried out in this direction worldwide, and on the other hand social-economic and demographic processes, occurring in Georgia.

Developed pension system format includes a combination of PAYG (pay-as-you-go) pension system and financial defined contributions model, which demonstrates the importance of the participation of public and private sector in the solution of the problem, will reduce adverse risks and prevent tension of the principle of solidarity between generations.

The introducing such laws should be promoted in Georgia, which provide the employment of the elderly population, as the age of the person should not be the reason for the discrimination of an individual.

Blood represents extremely difficult, special, vital system in many respects defining integrity of an organism. Blood - as a tissue, has   a number of features. High mitotic  activity of a haemo-poetic tissues causes its hypersensibility to action of damaging factors, and genetic determinancy of reproduction, a differentiation, structure and a metabolism of blood cells create preconditions, both for genomic infringements, and for changes of genetic regulation. The blood originality is in that the pathological changes in it arise not only owing to infringement of hemopoesis system functions, but also other internal organs and organism’ systems as a whole. 

In work the authors had given the modern literatures data on a platelets’ structure and functions, on wich analysis’ basis    authors suppose platelets’ immune competence.

RISK FACTORS OF CARDIAC BIOMARKERS ELEVATION AFTER CORONARY INTERVENTION IN PATIENTS WITH STABLE ANGINA

88 patients with stable angina and successful PCI procedures were studied at Cardiology Department of TSMU A. Aladashvili Clinic. The study revealed, that cardiomarkers elevation after successful PCI procedure was detected in 23.9% of patients. Cardiomarkers elevation was associated to the patients’ age, presense of arterial hypertension and diabetes mellitus. The risk factors of markers elevation were following laboratory tests: high levels of total cholesterol and LDL cholesterol. Cardiomarkers elevation is also asocciated to Gensini’s index, two vessels damage, the number of implanted stents and stenosis of LCX.

RELATIVE EFFECTIVENESS OF TREATMENT OF CARDIOSPASM WITH PNEUMOCARDIODILATATION, OPEN TRADITIONAL AND LAPAROSCOPIC SURGICAL METHODS

This study reviews treatment of cardiospasm with pneumocardiodilatation, open traditional and laparoscopic surgeries. The aim of the study is to compare the relative effectiveness of the treatments. The treatment results of 189 patients are reviewed (1993-2014yy), including: 70 cases of cardiospasm treatment with pneumocardiodilatation, 76 cases of open traditional surgery and 43 cases of laparoscopic surgery. Type of surgery used was Heller myotomy and Dor fundoplication (both open and laparoscopic methods). Optimal results were obtained in 71% of patients treated with pneumocardiodilatation. The optimal long-term outcomes with complete removal of dysphagia were obtained in 93.5% of patients treated with open traditional surgical method and in 95.4% of patients treated with laparoscopic surgical method. In terms of quality of life, fast recovery and cosmetic effect the laparoscopic approach has indisputable advantage. As our results suggest, the benefits of laparoscopic surgical approach are eminent and is the choice of method in treatment of cardiospasm based on our clinical experience.

MICROBIAL-INFLAMMATORY DISEASES EXCRETORY SYSTEM IN TEENAGERS OF CHERNIVTSI REGION

The objective. To reduce the regional features of prevalence and morbidity microbal-inflammatory urosiss among the children of teens the Chernivtsi region.

Patients and methods. Official statistical figures (current documentation about the state of nephrological help to the children of the Chernivtsi region for period from 2008 to 2012 and data of Center of medical statistics of ministry of health of Ukraine in relation to morbidity and prevalence of illnesses of microbal-inflammatory urosiss among child's population) are studied, for treatment the research and information is used, statistical methods.

Results. For last five-year period a most peak of registration of microbal-inflammatory diseases in area of was on 2011, high-performance among the next administrative districts of the Chernivtsi region: Vizhnitskiy district (in all it is registered diseases - 1186,0 on 10000 corresponding population), Zastavnyanskiy district (1057,9 on 10000 corresponding population), Kelmenetskiy district (1474,4 on 10000 corresponding population), Sokiryanskiy district (3466,4 on 10000 corresponding population), Khotinskiy district (1717,7 on 10000 corresponding population). At the same time pay attention on itself subzero indexes in Glybotskiy district (799,6 on 10000 corresponding population) and Novodnestrovsk (816,8 on 10000 corresponding population), among reasons is an insufficient clinical and laboratory orientation of work foregoing administrative territories (certain percent of diseases remained not diagnosed).

Conclusion. The research will allow to hold the correct and reasoned medical and rehabilitation events in all stages of rendering of specialized medical care for teenagers and to develop the programs, directed on improvement of indicators of health and prevention of disability in children.

THE STUDY OF THE THERAPEUTIC EFFECT OF PLAFERON-LB
IN PATIENTS WITH ACUTE MAXILLARY SINUSITIS

Difficulties of satisfactory diagnostics and treatments of paranasal sinuses's acute inflammation are wellknoun. Therefore, the aim is was to study treatment effectiveness of immune drug plaferon-LB, while puncturing it directly (needle Kulikovskii) into highmoritis sinuses of 44 patients in age from 8 till 62, who had paranasal sinusitis.

The diagnostic of this whole process and it's results were assessed as well as clinical and paraclinical methods of survey, among them is the method of electric paramagnetic resonance (EPR), which helps to fix the different changes (in examples) with different density. 

The studies showed, that the most effective method in this treatment of acute maxillary sinusitis is combined use of plaferon-LB with other drugs – antibiotics, more than using them separately in monotherapy form. The effect of this combined treatment gives the result of much more earlier and faster recovery of sick person.

In this case the positive progress was done, because of plaferon's high immunity ability, among them antioxidant action on cell level in mucous membrane of paranasal sinuses.

The obtained results were confirmed with both methods – clinical and preclinical. All these materials give opportunity not only to improve the diagnostic, but to prevent them.

Blood rheological status and Influence of grape seed extract in patients with parodontitis

Thus, in patients with parodontitis blood rheology and coagulation changes are present while there is endothelium dysfunction and this is a very significant problem. It does not still cause the degradation of body continuity although they are involved in risk groups of cardiovascular problems. Research measures of rheological parameters are not largely implemented in dental clinics of Georgia as well as pharmacokinetic and therapeutic effects of grapeseed oil are not studied enough. With the present work we tried to show how significant is the assessment of blood rheology in parodontitis, since rheological changes on one side play a very important role in the development of parodontitis and on the other side parodontitis causes abnormality of rheological characteristics and the use of grapeseed oil we considered as a way out of this vicious circle, as we showed in our previous article. Future research in this direction is really important in order to improve treatment tactics of parodontitis. It has propaedeutic as well as economic benefit.

PROVIDING THE ELDERLY POPULATION IN GEORGIA WITH QUALIFIED MEDICAL SERVICES

The article presents the issues of medical services for the elderly population, which are related to the global aging trend in the world.

Publication demonstrates the efforts of the international organizations for tackling the mentioned problem. A number of European countries practice at-home services of the elderly patients. In the big health clinics of the USA, Canada, Japan, Israel, Britain and France, geriatric units, daytime hospitals and at-home service systems have been functioning since the 80s of the last century.

The comprehensive medical service for the elderly is a topical matter in Georgia, due to the non-existence of the qualified geriatric personnel. Universal health insurance system has been operating since 2013 and a special state fund was created to finance it, but it would be desirable to pay attention to the preparation of the geriatric personnel (doctors, nurses, social agents) as well.

The article aims establishing outpatient clinic with day hospital and at-home service system of Geriatric profile at first stage in Tbilisi, with utilization of the material and professional resources existing in Georgia. It will serve one or two districts of the city, where it will render the first outpatient services, medical aid, psychological, social and legal support to the population aged 65 years and over.

The thyroid gland disorders are the contributing factors to the development of periodontitis. Despite the lack of works, discussing the hemoreological and calcium, phosphorus homeostasis disorders, resulting in the destruction of the tooth hard tissue mineralization, their authors confirm the unity of the microcirculation disorders and thyroid dysfunction. Our data also confirm this statement. We believe that the imbalance, caused by parathormone, leads to microcirculation disturbances not only locally, but also to all organs and tissues.

The paper reviews the information on a role of oral epithelial layer, gingival and pulp structural components, cellular composition and their innervations according own experiences and data, aviable in the literature.

In modern dentistry the problem of pulp innervations and its depending on the demyelination has assumed a particular significance.

The paper considers the main lines of oral mucosa damages wich act on the dental pulp inflammatory response, followed by the endothelial dysfunction and progressing demyelination.

OZONE AND OZONATED WATER IN MEDICINE

Ozone is an unstable gas comprising of three oxygen atoms, the gas will readily degrade back to oxygen, and during this transition a free oxygen atom, or free radical is form. The free oxygen radical is highly reactive and short lived, under normal conditions it will only survive for milliseconds. O3 used in various field of medicine for treatment of different diseases. Researchers   have shown the efficacy of ozone in eradicating a wide range of bacteria, bacterial spores, fungus and viruses.

Currently ozone is widely used for cleaning water systems and also ozone water used as a antiseptic in a treatment of different infection.  We have to use ozone water in paradontology, to compare it with other antiseptics and also to optimize medicinal dose. Information about this research will be in the next article.

The article summarizes the literature and our own data, dedicated to the study of the role of natural zeolites in biology and medicine. The positive effect of zeolite on mineral metabolism in animals and humans was revealed. Presents convincing data about radioprotective properties of the zeolite in an organism and the external environment, as well as the use of zeolites as a dietary supplement in various pathologies.

Kagocel possessed immunomodulating effect for interferon system in healthy volunteers. The inclusion of drug in the complex treatment of herpes of lips and mouth contributed significant acceleration relief of infection and prolong remission. Kagocel enhances the activity of endogenous interferon in patients and makes recommendations to the wider use of the drug in viral infections.

The present study was directed to investigation of β-cells ultrastructure changes in the rats pancreas islets during Alloxan diabetes. It was shown that the pathological processes detected the injuries in β-cells ultrastructure (dystrophic and necrotic changes of the β-cells). The changes developed in  the rats’ pancreas β-cells during Alloxan diabetes were strongly correlated with hyperglycemia. Data indicate the principal role of the β-cells damage during the diabetes mellitus manifestation.

Rehabilitation influences of adaptive physical exercises are based on interralation of impellent and vegetative functions. In particular, at corrective picked up physical exercises, according to functionalitis of an organism, muscular activity causes positive changes in work of internal bodies.

Received data of researching and control group, allows conclude, that the complex adaptive exersises and correct by selected power of physical load, with the accounting of time, cause comparative rehabilitation  muscular system for people with congenital aberration of genes, which is very important for their vital activity, from our point of view.
