White Coat Hypertension describe the well known reality – there is strong correlation  between high arterial blood pressure during medical check up. This fact leads over diagnosis and more drug therapy then it have to be.

 To fix the problem of  WCH  is very important for improving primary health care and make easy to diagnose arterial  hypertension earlier, and emotional liability as well. This is the major principle for effective prevention and treatment of AH.

COMPUTERISED DUAL TASK COORDINATION IN ATTENTION DEFICIT

HYPERACTIVITY DISORDER (ADHD)

The deficit of executive functioning was found to be associated with attention deficit hyperactivity disorder (ADHD) in general and its subtypes. One of the important functions of central executive is the ability simultaneously coordinate two tasks. The study aimed at defining the dual-task performance characteristics in healthy children and adolescents on the computerised dual-task method; investigating the effect of task difficulty on dual-task performance in ADHD in comparison to age and years of education matched healthy controls; investigating whether the dual-task functioning in ADHD is defined by comorbidity factors.

The study investigated dual task functioning in 6-16 years old 91 typically developing controls and 91 children with ADHD. It was found that: (1) the dual-task coordination is available in children and adolescents with ADHD in general and in its subtypes and not significantly different from performance of age and years of education matched healthy controls; (2) Increase of the task difficulty in dual-task paradigm don’t affect disproportionately children and adolescents with ADHD in comparison to age and years of education matched healthy controls; (3) The dual-task functioning in ADHD in general and in its subtypes is not defined by the comorbidity factors.

Femoral artery access for endovascular treatment of abdominal aortic aneurysms can caused hrombotic complications in the early postoperative period. In the article present the clinical case of hybrid surgical treatment of abdominal aortic aneurysms and severeis chemia of the lower limb in patients with concomitant diseases of high risk. During the operation uses a modified method profundoplasty of  femoral artery bifurcation.

HCV MANIFESTATIONS ON SKIN AND MUCOUS MEMBRANE

Most patients with chronic hepatitis C virus (HCV) infection are asymptomatic. Patients and health care providers may detect no indications of the conditions for long periods; however, chronic hepatitis C infection is slowly progressive diseases and result in severe morbidity in 20-30% of infected persons. Astute observation and integration of findings of extrahepatic symptoms, signs, and disease are often the first clues to underlying HCV infection. HCV is a major public health problem because it causes chronic hepatitis, cirrhosis, and hepatocellular carcinoma (HCC). Chronic hepatitis C infection is associated with many extrahepatic manifestations in joints, muscles, neural and gastrointestinal tissues, and skin.

In this article, the many dermatologic manifestations of HCV are classified according to diseases with proven or suspected etiology or causation.HCV induces extrahepatic manifestations such as mixed cryoglobulinemia, porphyria cutanea tarda, leukocytoclastic vasculitis, lichen planus (LP), and sicca syndrome, all of which should be regarded as early markers of a potentially fatal chronic liver disease. Other commonly encountered dermatological disorders linked with HCV infection globally include urticaria, pruritus, thrombocytopenic purpura, and psoriasis.

CREATION OF A STANDARD SOURCE FOR A VISIBLE LIGHT AND DETERMINATION ITS PHYSICAL PROPERTIES FOR STUDY OF AN INFLUENCE

A wide progress of a photodynamic and laser therapy stimulates a study of an influence of a visible light on human’s skin and hair. The study aimed to create a standard source of a light making it possible to obtain a stable visible light of different frequency spectra – white, red, green and blue. A matrix of four light diodes was used asasource of a light allowing to create almost monochromatic emission of a proper spectrum without any parasitic, infrared or ultraviolet emanations.

The device constructed was examined in experimental conditions by means of paramagnetic resonance for a study of an influence of a visible light on free radicals’ processes in dark hair of a human.

The activity of catalase, content of free NO and deformability of peripheral blood erythrocytes in patients with middle severity periodontitis were investigated under the influence of laser treatment.

It was revealed that in peripheral blood erythrocytes of in patients with middle severity under 7 days laser treatment erythrocytes deformability and free NO content did bot chainged in comparision of control level, catalase activity was increased. That indicates on antioxidant effect of laser treatment.

CHEST (THORACIC) BLUNT TRAUMA – SURGERY TREATMENT

Thoracic trauma is considered as one of the actual problems in surgery. Among the different injuries of the body it is distinguished with its solid specific gravity (12,5%) and severe outcome. In most cases the mentioned trauma can cause severe injuries of living organs in thoracic cavity resulting in high lethality (up to 20-35%). Complications and death are caused not only by the severity of trauma but by making a wrong diagnosis and incorrect tactics of treatment.

 The purpose of the present study is to introduce and share our experiences, based on the evidences of 312 patients having entered at the National Medical Center with chest closed trauma from 20012 to 2015. Among them were: men 216 (69,3%), women 96 (30,7%). Patients’ age varied within 15-78 years. 70% of the injuries were isolated and 30% - combined traumas (abdominal cavity, head, and extremities). According to the severity of the injuries of inter-thoracic organs the patients were divided into the following subgroups: I group – mild 128 (47.0%) patients, with lung contusion; II group – severe 102 (32,7%), with spread atelectasis and distress syndrome; III – 6 (1,9%) patients with the injuries of trachea and bronchus; IV – 12 (3,9%) patients with affected organs in mediastinum area (esophagus, veins, aorta and heart); V – 65 (14,5%) patients with endopleura bleeding, caused by the injuries in blood vessels.

 The analysis of the cases showed that making of specified and timely diagnosis of the injuries of thoracic organs at cases of closed trauma seems to be very important, as the rate of effectiveness after surgical intervention at the injuries of blood vessels, esophagus and tracheobronchial tree came up to 51,1% instead of late intervention (36,2%), respectively. The rate of lethality has been reduced from 62,3% to 48,9%, respectively.

Finally, in the first 24 hours a surgical intervention was required in 37 (10,6%) patients with chest closed traumas; in 1-5 days after trauma – in 16 (5,2%) of patients; due to the pulmonary complications 32 (10,3%) patients underwent planned operations; 227 (73,9%) were treated with conservative method; in 33 (15,6%) patients – lethality outcome has been observed, respectively.

According to the above-mentioned it should be concluded that complex and consecutive approach to the investigation of the chest trauma, including Computer Tomography together with X-ray and Endoscopic studies, gives the possibility to specify the injury area at early stage of its setting and choose the appropriate method of adequate treatment and respectively, timely perform surgical intervention. Due to using of aggressive and selective bronchoscope insufflations methods and reduction of lung artificial ventilation, pulmonary complications have been decreased from 16,9% to 10,3%. The high effectiveness (>88%) of planned surgical treatment of mentioned complication showed that the date of surgical intervention should be chosen very carefully and done after localization and general stabilization of the mentioned disease.

MORPHOLOGIC BASIC OF EARLY SPONTANEOUS ABORTION

Retrospective analysis of the early spontaneous abortions has been conducted /486 cases/. Histo-morphologic analysis of the curettage material contents revealed involutive-regressive developmental changes in 108 cases, in 370 cases (77,78%) pathologic changes, like inflammatory changes of decidua and chorionic villis in 303 cases (80,16%), pathologic prematurity of  chorionic villi in 47 cases (12,43%), hydatidiform mole in 28 cases (7,14%). Combination of morphologic changes are found in 13,49% of cases, inflammatory changes of  chorionic villi together with prematurity of the structures – 90.2%, also inflammatory pathology with the changes characteristic for the embryonic sac – 9,8%. Frequently pregnancy has been terminated in the critical phases of the placentation on the 7-9 week of gestation –  44%, 10-12 gestational week – 33%, 4-5 gestational weeks – 23%. Existing results are of great importance for the preconceptional preparation of women.

The activity of catalase, content of free NO and deformability of peripheral blood erythrocytes in diabetic patients with and without varying severity periodontitis were investigated. It was concluded that peripheral blood erythrocytes in patients with diabetes type: – Deformability decreased with increasing severity of periodontitis; – Catalase activity increased with the increading severity of periodontitis; – Free NO content increases with the severity of periodontitis.

BACTERIAL  FLORA  OF  DENTAL  PULP  ROOF  UNDER  ACUTE  AND  CHRONIC  PULPITIS

Comparison data of patient’s G and M immunoglobulins levels against Herpes, Rubella and Adenovirusantigens under different clinical cases of pulpitis are given in this raport. Index of phagocytosis and neutrophils number also were calculated.

It was shown that ELISA analysis indicates on the hight antiadenoviruse IgG-level vs herpes and rubella antibodies in patiens with some somatic histories.

Phagocytic Index and neutrophies numder also increased in this category of patients wich results on the significant complications during chronic pulpitis recovery.

SURGICAL TREATMENT OF CHRONIC VENOUS INSUFFICIENCY IN ELDERLY PATIENTS WITH CO-MORBIDITIES (METHOD CHIVA)

The aim of the study was to examine the effectiveness of the method CHIVA in elderly patients with co-morbidities.

Materials and methods. In the period from 2011 to 2015 treated 37 patients with CVI, treatment which has been selected CHIVA. The average age of patients was 78 years. Operations carried out under local anesthesia.

Results and discussion. All patients were observed regression of signs of CVI. In 23 (62.1%) patients achieved stable cosmetic effect when observed for 3 years. All patients in the postoperative recurrent bleeding from varicose nodes were observed. Paresthesia in the places of cut was noted in 14 (37.8%) patients, subcutaneous hematoma were observed in 3 (8.1%) patients. Thrombosis saphenous veins without signs of inflammation were found in the control ultrasound examination in 11 (29.7%) patients. Thrombosis of the deep venous system were observed. Patients with concomitant diseases systemic complications and exacerbations of the main chronic diseases in the early postoperative period was observed.

Conclusion.Using the method CHIVA therapy in the treatment of chronic venous insufficiency is a safe and effective method and can be used for patients elderly burdened with co-morbidities.

PROSPECTIVE EVALUATION OF THE CLINICAL EFFECT OF PROBIOTICS IN THE FORMATION OF INTESTINAL MICROFLORA IN INFANTS

Main goal of study is to evaluation of the clinical effect of probiotics (Lactoral) in the formation of intestinal microflora in infants.

76 children from 3 weeks to 12 months were held for the survey. Depending on the delivery of all surveyed children were divided into two groups: the first study group comprised 42 children born as a result of a caesarean section, the second comparison group comprised 36 naturally born children. Macroscopic characteristic of feces was assessed on a scale of Bristol.

Deficiency of lactobacterium and bifidobacterium (prior to the appointment lactoral) is particularly pronounced in children born by caesarean section compared with children born naturally. The frequency of occurrence of enterobacteria and staf. aureus and candida in this group concerning  comparative group is  high (p Â0.05).

The use of Lactoral in children (aged under one year) has a positive effect on psychomotor and physical development. Indicators of body weight, growth and psychomotor development close to the norm after Lactoral administrated in 80.9 % of babies born by caesarean section and in 94.1% of the children born naturally.

PARTICIPATION OF THE NERVOUS SYSTEM IN THE MECHANISM OF HYPER FERMENTEMIA

According to analysis of clinical and experimental data we can conclude that activation of nervous system, appearing after the pathologic process, independently  from it localization, determines the rising hyper permeability of cell membrane and excessive aminotransferase from cardial muscle and liver.

We can consider that pathologic changeable organ being more sensitive object of influence nervous system becomes also the main source in receiving enzymes in blood.

That’s why organ localization of pathologic process predetermines not only the activation of sympatheticoadrenal system, but also the nature of its results, specifically the degree and “organospecific” hyper fermentemia peculiarities.

LIPIDS OF THE SEEDS OF THE CUCURBITA MAXIMA DUCH GROWING IN GEORGIA

Qualitative and guantitative composition of phospholipids of the polar sum lipids of the  seeds  Cucurbita maxima duch, growing in Georgia was studied. Lipids in the  seeds is in amount 3,2%. There were detected it following phospholipids: lisophosphatidilcholine, phosphatidil-inosit, phosphatidylcholine, phosphatidilethanolamine, N-acylisophosphatidilethanolamine, N-acylphospha-tidilethanolamine. Quantitative determination of phospholipids was estimated by inorganic phosphore-spectrophotometrically at 820nm.

 The oil from the seeds of Cucurbita maxima contains of carotinoides 2,7mg% and the seeds of vitamin C is 0,11%. In the alcohol extract from the seeds the following  ethanolicaminoacids were found: phenylalanine, methionine, alanine, serine, asparagin, lisin and histidin.

Dirofilariasis is one of the transmissible zoonotic diseases, which is spread in many regions of the world in canines. Sporadic cases of  human infection have been registered in almost every region. Thus, hypodermic dirofilariasis is to be considered in differential diagonosis in cases of hypodermic inflammations.
