CHRISTIANITY AND BASIC ASPECTS OF THE DEVELOPMENT OF THE

Christian medicine is multifaceted and at the same time bifacial. Beingmultifacetedis conditioned by different kinds of ethno-medicine generatedin the geographical area of the whole world (Chinese, Indian, Mesopotamian, Mediterranean, American and African Aboriginal, etc.) while being bifacial is defined by its rational-irrational nature. However, this fact can be considered to be conditional as it becomes increasingly difficult to draw the line between rational and irrational. Especially, if we consider the final result of treatment to be the starting point.

The basic characteristic of pre-Christian medicine is medical-intellectual systems mainly based on polytheist religion, where the mainstream was the Gods “heavy with human weaknesses” (envy, acrimony, vengeance, anger and love of gain). They were responsible for the disease-causing creatures (e.g. “Babari” in Georgian and Sumerian myths and folklore). Humans dominated by these wicked forces begged them for life and health like slaves. The attitude towards the diseased is clear: the ones who are bound to be survived are survivors while the ones causing danger to others are doomed. There is no desire for empathy, self-sacrifice for the diseased etc. In short, love is not takento the highest rank.

Christianity has brought a completely new approach among people and most importantly, the new attitude towards God himself. A human is no longer a creature pleading to the evil forces, an insignificant being. He is a man that God created in His own image and made him follow the path of righteousness, purification and love.

The concept of love not only for the neighbor but also for the enemy emerged. A man became an object of mercy and empathy. Jesus Christ whose miracles are mostly connected to a cure became a role model for Christian holy fathers, ordinary clergymen or sincere Christians. When getting familiar with the life of holy fathers their medical activities are obvious. There was no church or monastery where you could not find at least one clergyman with medical knowledge. Taking care of the diseased and the sacrifice for them became the way of life for a true Christian.

EFFECTIVE PREVENTION OF BREAST CANCER RECURRENCE: THE ROLE OF EXOGENOUS PEPTIDES MHP IN RESTORING IMMUNE HOMEOSTASIS

The article reveals the current state of the problem of early recurrence and metastasis of breast cancer (BC) and provides basic molecular and cellular mechanisms of how tumor cells avoid the immune surveillance system. The article stresses the importance of the phenomenon of epithelial-mesenchymal transition in the ability of epithelial cells to invade surrounding tissues and metastasize to distant organs, as well as focuses on circulating tumor cells as the main factor of unfavorable prognosis of the disease. We present the results of integrating Ñamelyn-Bio – product made of exogenous peptides – in the scheme of management of breast cancer. In the group of patients receiving exogenous peptides we observed harmonization of immune homeostasis, manifested by increased immune surveillance of the tumor process involving activated T cells, and increased expression of TNF-á and IL-12. Within 18 months of observation in 95.8% of patients treated with Camelyn-Bio we haven’t observed recurrence and metastases, while in the comparison group, this index was 89.6%. These clinical data demonstrate the effectiveness of Camelyn-Bio in harmonization of immune homeostasis in patients with breast cancer, which leads to increased immune surveillance of the tumor process and consequently prevents recurrence and metastases.

STUDY OF THE EYE FUNCTIONS IN PATIENTS WITH TUBERCULOSIS

The eye functions of the patients receiving medications for multidrug resistant and regular tuberculosis treatment, should be tested and examined. The visual acuity should be assessed by Snellen chart, color perception should be tested by Ishihara test. The fundus of the eye should be examined using Goldmann indirect goniolens and ophthalmoscope during and at the end of the study. While receiving Linezolid and Ethambutol, the toxicity of these drugs reduces the visual functions and causes alterations of fundus of the eye. This shows that the dosing of medications mentioned above should be managed and controlled the way that patient’s visual functions and fundus’ normal condition be preserved.

Rosacea is a skin disease, which was known as disease of the central part of the face, affects people of both sexes and is manifested by the persistence of erythema. The receptor apparatus located in the skin cells has a great influence on the choice of therapeutic measures for VFR. It is the increase of the androgen receptors in the epidermis and the epithelium of the sebaceous glands with endogenous adrenal stimulation in women, possibly leading to hyperplastic processes of the sebaceous glands. The aim of the study was to determine the IHC diagnostic criteria for the prescription of the disease and the possible dependence on the histological features of the rosacea vascular forms of the age of the patients. As primary, monoclonal antibodies were used for COX-2, AR and ER. The authors structured the early and late histological characteristics of skin lesions in vascular forms of rosacea, identified the main significant diagnostic criteria for this subtype of the disease, and analyzed their dependence on the age of the patients.

Modern literature sources allow attributing this disease to the group of angioproliferative diseases, which are based on the activation of inflammation and angiogenesis [1,3,9,11]. Rosacea is diagnosed when one or more of the following primary features are present: flushing (transient erythema); persistent (non-transient) erythema in certain facial areas; papules and pustules; and telangiectasia [9]. The available conventional medical therapies for rosacea are capable of counteracting augmented innate immune response by inhibiting steps involved in cathelicidin processing, it is not entirely clear why these agents would not reduce diffuse facial erythema. It may be because these agents are initiated after structural changes in cutaneous vasculature have already become fixed [3].

VAGINAL CUFF HEALING AFTER LAPAROSCOPIC AND VAGINAL HYSTERECTOMY

IN MORBIDLY OBESE WOMEN

The aim of the study was to compare postoperative vaginal cuff healing process after laparoscopic hysterectomy (LH) and vaginal hysterectomy (VH) in morbidly obese patients (BMIe”35kg/m2).

For this purpose, a multi-center prospective research was conducted in patients with benign pathologies, who underwent LH (n=25) and VH (n=29). In both groups patients were examined vaginally in 8-weeks postoperatively; vaginal cuff healing process was evaluate as: a) complete or b) incomplete. The study was conducted for the period of January 2014 – November 2017.

In LH group – 80% of patients had complete cuff healing (20 patients), 20% incomplete (5 patients); in VH group – majority of patients (n=28) had complete healing (96,4%) and only 1 patient had complete (3,6%).

According to the study, it may be concluded that in morbidly obese patients in 8-weeks post-surgically vaginal cuff healing is better after VH, compared to laparoscopic one.

ASPECTS OF PALLIATIVE CARE IN POST STROKE SYMPTOMS

Stroke is characterized with high lethality and is one of the main reasons for long-term disability, which makes end of life care and palliative treatment essential.  Today, it is unclear what kind of end-of-life care that patients dying from stroke receive. Do they receive care equivalent to that of patients suffering from the “traditional palliative care diagnosis”, cancer and AIDS? That’s why we believe that the problem is very actuals.

Background.  To determine the frequency of post-stroke residual events, analysis of the current tools used during symptoms management process and finding the new ways for their further improvement.

Methods. 196 patients were under palliative care: 86 women and 110 men, at Therapeutic Clinic of Georgian Patriarchate from 12.02.2010 to 01.01.2018. Age ranged from 42 to 97 years; The total number of bed-days amounted to 2588, on average patient stayed in the clinic for 13.2 days. Out of 196 patients who had a stroke, 124 died, or 63,3% of all cases were lethal. 35.5% of patients had a hemorrhagic type of stroke, and 64.5% had a brain infarction (P<0,005). 40% of the patients were in the clinic for 1 to 5 days, the mortality of whom was almost 100%.

Before hospitalization 156 out of 196 patients were diagnosed with various sizes, depths and degrees of Decubital ulcers in different parts of the body, which was the main cause of infectious complications in patients with asthma and weakened immune system. One of the most frequent complications of post-stroke residual effect was shortness of breath, which is why patients required oxygen supply. 144 out of 186 patients were in need of constant oxygen supply. It should be noted that 125 patients had tracheostomy, 165 nasogastric tube, and all patients had urine bladder catheters.

Results. High percent of development of decubitus ulcers were: various disability: hemiplegia, tetraplegia, hemiparesis, tetraperezis; different stages of abnormal consciousness and mostly comatose condition; disorders of blood circulation according on accompanied diseases. We think, that, we need to improve quality of care. Patients had accompanying diseases: Essential hypertension, Congestive heart failure,   Diabetes mellitus, Atrial fibrillation. The listed diseases require special management. Permanent monitoring and timely correction of vital indicators is necessary.

Conclusion. According on our study and analyses of literature can be assumed, that timely and adequate assessment of symptoms and their timely correction is a best way for improvement of the patients life quality. For this purpose, we believe, that is necessary to make guidelines for Georgian patients including our traditions and culture and special study of palliative staff.

In the article the etiology and pathogenesis of chronic bacterial prostatitis is observed and attention to the increased risk of neoplasia in patients with chronic inflammation of the prostate is accentuated.

Also, the article reveals mechanisms of action of exogenous peptides in the process of harmonization of immune homeostasis,in which tissue accumulation effect plays a crucial role. Compared with the reference group, the integration of Camelyn-Bio in management of chronic bacterial prostatitis leads to better effectiveness of treatment, as evidenced by the clinical data.

Aim and objectswas to study peculiarities of young pharmacists in Georgia. The study was quantitative investigation by using survey (Questionnaire). Research objects are materials of sociological research: Surveys was for young pharmacist specialists, 314 young pharmacist specialists were interviewed in Georgia. We used methods of systematic, sociological (surveying, questioning), comparative, segmentation, mathematical-statistical, graphical analysis.  The data was processed and analyzed with the SPSS program.

In the article, there have been studied the selenium spraying operations during spleen trauma, and the authors go to the conclusion that the organ’s maintenance operations are advisable  to  carrying out in all cases when it gives opportunity to preservation the spleen tissue. The authors have carried out the research the immune status of that patient who carried out the economical surgical  interferences. Based on  the results of the analysis  it’s possible to make decision that  after  two weeks the operation the patient’s immune status of the major indexes have been reverted  (came back) to evidences of control group or differ vary slightly from them. This fact suggests that the deviats in immune status in the second day of operation have been reveals  only in case of  after surgical aggression. Thus, based on above mention it follows logical conclusion  about expediency  to carrying out the economical operation. Also, it should be noted that after period of operation no one patient did not death.

THE IMPORTANCE OF THE GALLBLADDER VEINS IN LIVER ONCOSURGERY

In the article ,there is a  consideration of the question  of autonomizing the portal area of   the liver (Lobe, sector, segment) in norm and with liver cancer. Materials for research and methods are sufficient to solve the presented tasks .

On the basis of research  is established that the veins of the gallbladder are associated only with the portal system And also there are additional portal veins,which are found in 64.2% of cases.Among them one vein -accidentally detected in 39,51%,two veins in  13.02,%,three veins in 6,0%.four veins in 4,1%,and five veins in 1,4%.The veins that were listed had a caliber 1-5 mm. These veins are joint vein’s branches of  right and left  lobe of the liver  to gallbladder .

This gives us an opportunity to think that during partial or complete blocking of the vein by a tumor,additional portal veins and veins of gallbladder can provide  compensatory bloodstream circulation of the portal vein in the blocked part of the liver.

Thus, the collateral circulation system allows  an organ, which is affected by a tumor, to carry out internal dissemination from one half to another.

HEREDITARY HEMOCHROMATOSIS  IN COMBINATION WITH

HOMOZYGOUS  b-THALASSEMIA

Hereditary hemochromatosis (HH) is an autosomal recessive disease associated with a violation of iron metabolism that leads to its increased accumulation in organs and tissues. The results of the phenotypic and genetic frequency survey in the Azerbaijan Republic population showed existence of 12,3% of the phenotypic and  0,1865% of  genetic frequency  of hereditary hemochromatosis among the people surveyed.

Wide distribution of b-thalassemia in Azerbaijan with the simultaneous presence of the hereditary hemochromatosis gene in a number of cases creates favorable condition for emergence of double carrier in these genes, that significantly worsens the course of the disease. We described a b-thalassemia-affected family which was hereditary hemochromatosis carrier.

We studied biochemical and genetic aspects of this pathology. We revealed a high content of iron indicators and its components. Family - genealogical method established the hereditary character of these two pathologies.

 The data obtained can improve the diagnosis and treatment of families double carriers of hereditary hemochromatosis and b-thalassemia.

ELECTROPHYSIOLOGICAL GROUPS OF O-LM, PERISOMATIC AND TRILAMINAR INTERNEURONS IN CA1 MICE HIPPOCAMPUS

Cortical interneurons are very diverse with their morphology, biophysical, molecular and electrophysiological properties. The aim of presented work was to find out the reliability of parvalbumin content as a marker of fast spiking non-accommodating  interneurons and to investigate whether all PV-containing cells belong to the morphologically and electrophysiologically same type of interneurons.

For this reason hippocampal interneurons were recorded from C57Bl6 and transgenic mice with EGFP combined with PV promoter with the help of whole cell patch-clamp technique. Patch pipettes were filled with biocytin for staining. After reconstruction the cells were grouped morphologically into trilaminar, perisomatic and oriens-lacunosum-moleculare interneurons (O-LM). Then they were classified according to electrophysiological properties in response to various steps of depolarizing current injections. It appeared that in healthy hippocampus there are two types of O-LM cells: fast spiking non-accommodating and accommodating ones. And the former contain PV. The discharge frequency of NAC O-LM cells is similar to that of trilaminar interneurons and exceeds firing frequency of perisomatic cells. This investigation shows the novel type of O-LM interneurons and indicates that parvalbumin, once considered as a classical marker for fast spiking non-accommodating  perisomatic interneurons should be substituted with other more cell-type specific markers.

Pruritus is the most frequently described symptom in dermatology and can significantly impair the patient’s quality of life. In 10–50% of adults with persistent pruritus, it can be an important dermatologic clue for the presence of a significant underlying systemic disease such as renal insufficiency, cholestasis, hematologic disorder, or malignancy.This review describes the presence of pruritus in different systemic diseases.

It is quite important to discover the cause of pruritus for providing relief for the patients experiencing substantial morbidity caused by this condition.Systemic causes must be considered, especially in elderly patients in whom pruritus is persistent and refractory to xerosis management and other nonspecific therapies. Therefore, a thorough history and physical examination is essential in the evaluation of chronic pruritus.

A multidisciplinary approach with a dermatologist, psychiatrist, and internist is required to prevent psychiatric morbidity and possible deterioration of quality of life in these patients.

Pruritus therapy can be topical or systemic, symptomatic and causal. In many cases it is necessary to avoid excessive bathing, irritative fabrics, vasodilatation caused by alcohol, hot liquids or foods and stress. In some cases, patients can use antihistamines and / or sedating drugs. Tricyclic antidepressants are also useful. The most important tool for treatment of the pruritus in internal diseases is the specific treatment of the concrete internal disease, which should not be underestimated.

EFFECT OF  E.COLI PHAGE LYSATE VACCINATIONS ON IL-12 ANDIFN-ΓSECRETION AND PARAMETERS OF CANCER GROWTH IN LAB MICE WITH EHRLICH CARCINOMA

Anticancer treatment effects of E.coli phage lysate vaccinations (0,25 ml, 1x intraperitoneally, with 5 days interval) have been studied in 2-3 months old 30 lab. mice with Ehrlich carcinoma. The treatment efficacy was estimated by the dynamic of growth of cancer tissue and cancer growth inhibition percent. Immune status was studied by measurement of IL-12 and IFN-γ concentration using  ELISA. Investigations have shown that E.coli phage lysate injections delay the cancer growth. Anticancer treatment effect was especially obvious and well expressed in case of 3and 4times vaccinations. During the same period increased secretion of IL-12 and IFN-γ in treated mice compared to the untreated and healthy control was detected. After 5-8 injections the anticancer action of the preparation decreased.Prolonged vaccinations supported progressive cancer growth. Concentration of IL-12 and IFN-γwas decreased as well. The peak of cytokines’ secretion was reached after 3-timesadministration of phagelysate, but after 5-8 vaccinations there were no statistically significant difference between results of experimental and control group animals. Could be concluded that the bacterial preparation - E.coli phage lysate reveal anticancer immunomodulatory properties. For positive treatment results the detection of optimal dosage and regimen of vaccinations is important.

Metastases in the neck occur mostly due to a primary tumor in the head and neck region. Surgical treatment of the neck is the treatment of choice for resectable disease i.e. various types of neck dissections. In current oncology practice there is great emphasis on organ-saving and function saving procedures that suit to the head and neck region too. Selective neck dissections are in common practice in cancer clinics due to the good understanding of the patterns of metastatic spread to various neck levels. This publication emphasizes on the need for adapting the internationally accepted classification of neck lymphnodes in oncology practice.

Our research is one little, but significant segment of the great research, it is the beginning stage of the great research process and its goal is to draw up the criteria for oral cavity health condition assessment. These criteria will be used in further researches and based on them will be implemented the oral cavity health condition assessment of the patients.

As a result of the research the following criteria were chosen: I. CFE-index: 1. satisfactory, 2. non-satisfactory, 3. low quality; II-gingivitis index: 1. light, 2. moderate, 3. hard; III-periodontal index:  1. light, 2. moderate, 3. hard.
