Modern model of Simulation Based Medical Education  (SBME) is relatively new method of learning used for a long time in  high risk professions, such as aviation. The transfer of lessons from such industries is due to very low failure rate, while medical errors call for a change.  To learn skills in safe environment is very important.

However hundreds of years ago complex models were used to help teach anatomy obstetrics, surgery and many other disciplines. The newest technologies were often used in simulators to improve fidelity. The advantages of this mode of learning are already revealed by several studies, though there are some barriers to the implementation of SBME.  Nowadays research in this area of medical education is limited, more studies are need.

MODERN VIEWS AND METHODS OF DEVELOPING SOCIAL PROGRAMS IN GEORGIA

Methods of developing social security programs in Georgia and contemporary visions are still an important problem. There is no comprehensive model in the development of social sphere programs and some of the principle can be used in a number of countries.

According to research conducted by international organizations, social protection should include programs prepared by the principle that protect individual (household) from two unfavorable results: permanent poverty (permanent inability to earn income) and temporary poverty (temporary absence of minimum subsistence).           

On the development of modern visions, methods of implementation of social programs in Georgia and realization of subsequent practice - the main accents of the research are determined and based on the following factors: Poverty and quality levels, and target groups to determine the composition of the group within the administration the opportunity, political and economic factors and the financial investment  timely assistance and livelihood as well as the rate (household) survey shows a causal connection, the core problems of the description socio-economic approach. With international experience, even in different regions of the same countries, the method may have different impacts on the population of the poor and cause various consequences. The complex assessment of income and livelihoods in the transitional economy in Georgia is not practically impossible, based on our analysis of the research carried out by us - for 19 reasons. All are given and detailed in the article.

CORRELATIONS BETWEEN SPONTANEOUS ABORTIONS AND PATIENT’S HISTORY, SOCIAL-BIOLOGICAL AND ENVIRONMENTAL FACTORS.

We retrospectively studied the clinical and laboratory features of 111 patients with spontaneous abortion from the contingent of the National Medical Center O.Gudushauri and the First Medical Center, and who were treated in 2016-2018. Data from 103 pregnant women who gave birth at a time (9 months) in the same period of time were taken for control.

With spontaneous abortions, a significant positive correlation is manifested in: housing and labor conditions, mode of diet, gynecological analysis.

As it is known, over past twenty years  decrease in gender trends is underway. This fact was confirmed by the results of the previous survey. At this pace, Georgia will take many years to achieve gender-neutral birth.

SPECIFICITIES OF PTEN EXPRESSION IN NEOPLASTIC PROCESSES OF ENDOMETRIUM

Introduction. Tumor suppressor genes, i.e. so-called anti-oncogenes prevent the malignant transformation of cells in the body.  As a rule, they can be detected only during inactivating mutations, which phenotypically are revealed by formation of tumors. The subject of our interest is the gene PTEN. Inactivating mutaions of this gene has been detected in different neoplastic pathologies. In endometrium, as highly proliferative and cyclically regenerative tissue, PTEN mutation appears to be a gatekeeper for endometrial carcinogenesis. The goal of our study was to explore the endometrial   hyperplasia/dysplasia/adenocarcinoma to identify the phenotypic characteristics by expresson of PTEN protein in each condition.

Material and methods. The study represents a retrospective research. The coded and depersonalized  material data from Acad. N.Kipshidze Central University Clinic was used in the study. 5 study groups with age distribution subgroups (-.1 reproductive age, -.2 menopause, -.3 post-menopause) were selectedfrom routine histopathology tissue specimens of uterus. Immunohistochemistry with marker PTEN was performed. 

Results. The study showed that: in 1st study group Endometrial Hyperplasia PTEN is diffusely positive with focal heterogeneity 11.1%. In 2nd study group Endometrial Dysplasia, PTEN is heterogenic in 18.3%, negative in 6.3% of cases. In 3rd study group Endometrioid Carcinoma G1, PTEN is heterogenic in 35.7%, negative in 14.3% of cases. In 4th study group Endometrioid Carcinoma G2, PTEN is heterogenic in 43.5%, negative in 30.4% of cases. In 5th study group Endometrioid Carcinoma G3, PTEN is heterogenic in 33.3%, negative in 41.7% of cases.

Conclusions. It was found that, heterogeneity peak of PTEN expression takes place in the cases of endometrial carcinoma G2, but the negativity of PTEN protein is increasing in parallel with the malignancy increasing process and reaches peak performance in cases of endometrial carcinoma G3.

EFFECT OF GALLERIA MELLONELLA EXTRACT ON THE DEVELOPMENT OF HERPES INFECTION IN WHITE MICE AND TISSUE CULTURES

The effect of the wax moth larvae (Galleria mellonella) extract on the presence of antiviral action in white mice and tissue cultures was tested, and the immune parameters of herpes infection were investigated. Compared with the control, the lethality of animals and virusemia significantly decreased. The protective effect of the drug was also manifested in tissue cultures. The extract was effective in activation of interferon, phagocytosis and production of antibodies.

It is concluded that the method of biogenic stimulation is a promising direction for correcting the results of anthropogenic pollution of living organisms.

In recent years there have been messages about appearance of chlamydia infection in the rectum. First of all it can be explained by ineffective treatment of urogenital chlamydia infection.

The use of helium-neon laser in complex treatment of postoperative lesions in chlamydial paraproctitis patients, reduce aseptic inflammatory and microcirculation disorders, accelerates the reparative process by increasing the level of proliferative activity and reduce hospital stay of patients.

The results of study shows, that using Lazer therapy after facial boneplastic surgeries when the biocompositive materials are used. Opt: mises the heeling proccess, and makes better the quality of treatment. Late results show, that using lazer therapy makes faster. Osteogenesis around the implant, and the rezorbtion is adequate.

According to the results presented, marriages among underage is still important challenge in the country. Basically marriage of 15-17 year old girls is common practice. The trend of marriage and facilitating factors of this period is closely linked to the gender gaps in the society, which has complex form and requires further studies.

According to the results of the presented research, high level of education remains the key factor of low birth rate in Georgia. There are also varieties of birth rate of regions. Low indicators remain in city-type settlements. Among the cultural characteristics, Religion  is a strong factor for high level of birth.

CHRONIC CALCULOUS CHOLECYSTITIS OR CANCER OF THE GALLBLADDER AND LIVER ABSCESS

We are presenting an unusual case of formation of pyogenic liver abscess due to intraparenchymal perforation of the gallbladder in the liver and formation of the fistula between the gallbladder and the liver abscess: Manifestation of the weakness, non-fever weigh loss, thick and rough gallbladder walls and a small mass in the liver indicating the tumoral process. Tumoral markers were negative. However ultrasound and CT scan showed gallbladder tumor and metastatic lesions of the liver and diagnosis was established at laparatomy. Open drainage of the liver abscess after cholecystectomy was enough for good outcome. In the literature, there have been very few reports of intrahepatic perforation of the gallbladder resulting in formation of hepatic abscess.

The presentation, diagnosis, and management of liver the abscesses, as well as gall-bladder cancer diagnosis, are discussed.

FEATURES OF THE IMMUNE STATUS IN PATIENTS WITH

VARIOUS CLINICAL FORMS OF ROSACEA

Despite the presence of many concepts trying to explain the mechanism of rosacea development, it must be assumed that none of the etiopathogenetic factors is universal for substantiating the pathogenesis of rosacea. Most likely, this disease is a certain combination of various factors that ensure the formation of a pathological process, where immunopathological reactions are far from the last, leading to an imbalance of the main protective functions of a person. The presence of Demodex Folliculorum mite, whose intensive reproduction is associated with a serious imbalance of immune parameters and is one of the frequent etiological factors in the development of rosacea, should be taken into account.

Based on the foregoing, the purpose of the work was to assess the immune status of patients with various clinical forms of rosacea, as well as to establish its connection with the prescription of dermatosis.

Under observation were 20 patients with rosacea (12 women and 8 men, aged 18 to 45 years) with disease duration from 1 to 10 years. The clinical forms of the patients were distributed as follows: erythematous-telangiectatic form - 8, papulopustular form -12.

Mite identification was performed by microscopy. The immune status of patients with rosacea was investigated by laser cytometry and chemiluminescent immunodiffusion methods. Lymphocyte subpopulations, activation markers by immunofluorescence using monoclonal antibodies and a flow-through laser cytometer manufactured by Becton Dickinson. The concentration of Ig G, Ig A, Ig M was determined by the turbodimetric method, the circulating immune complexes - by the turbodimetric method by the rate of formation of aggregates in 3% and 4% polyethylene glycol.

According to the results of our study of immune homeostasis in patients with papulopustular rosacea (PPR) with a high density of tick populations, a picture of a chronic immune response was revealed, in which T-cells - suppressors, T-cells - helper cells, and also antibodies of the G, A, M class actively participate. The detected changes in the immunogram indicate the intensity of cellular and humoral immunity. In patients with the erythematous-telangiectic form, changes in the parameters of the immune status were mainly related to cellular immunity parameters. When studying the immune status of patients with rosacea, it was revealed that the disease proceeds against the background of a noticeable immunological imbalance, which largely depends on the severity and duration of the pathological process. With an increase in the duration of the pathological process of the skin, there was a significant violation of the T and B immunity parameters and an increase in the concentration of circulating immune complexes.

Determining the immune status of patients with various clinical forms of rosacea allows the use of immunological parameters for early prediction of the outcome of the disease, possible recurrence of dermatosis, as well as assessing the quality of treatment of dermatosis.

Use  of  Probiotics in pediatric practice is very  actual problem in pediatric practice. According to literature, it was determinedSequential therapy combined with  probitics seems to be the best  efect. This review focuses on the efficacy of probiotics for diarrhea in children. Today, we have a large number of published clinical trials on the role of probiotics in treating sporadic infectious diarrhea in children, and many of them are randomized, blinded, and controlled.

They consistently show a statistically significant benefit and moderate clinical benefit of a few, well-identified probiotic strains – mostly Sacharomyces bulardii, L. Rhamnosus.

The specific clinical effects that each probiotic or prebiotic may cause must be considered.

POST-SURGICAL REHABILITATION OF CANCER OF THE UPPER JAW

Maxillary tumors make up 2-4% of all oncological diseases and 24% of head and neck tumors. This pathology is equally common both in men and women patients, aged above 40 years. Maxillary cancer was first discovered in the human skull of stone age. Famouse representatives of the medical filed, such as Freud and Pirogov were suffering from maxillary cancer. Upper jaw cancers are characterized by numerous symptoms. This symptomocomplex can by neurological, ophthalmic, otolarhingological and dental. To study the maxillary cancer it is important to consider (review) the relationship between the upper jaws and neighboring bones. For this purpose, we will briefly refer to the skull.

THE AUTOIMMUNE DISEASES AND HUMAN MICROBIOTA: NEW VISIONS

The development of autoimmune diseases is presumptively initiated by an aberrant, genetically regulated, immune response to environmental antigens. The combination of environmental and genetic factors determines the development and expression of an autoimmune disorders. Inflammatory rheumatic diseases are thought to be secondary to the loss of the mechanisms of immunological tolerance. 

The last decade has witnessed an explosion of research into causes and the consequences of alterations in the microbiota. The topic outlines the factors that may predispose to dysbiosis, altered microbiota and JIA. Growing evidence indicates involvement of the gut microbiota in the pathogenesis of JIA. Understanding of the interactions between the human host and microbiome will help to find new ways of treatment and prevention.

AGE-RELATED CHANGE OF PHYSICAL  PARAMETERS OF ERYTHROCYTE

Objective of the research was to study age-related changes of deformability of .erythrocytes. Studies have been done on five  age groups: 1. 17-25years old.2. 25-35  years old3. 35-604. years old 5. 75-90 years old. Base on the results, we can assume that in blood of younger volunteers to compare with deformability indicator, in peripheral blood erythrocytes of older people, has demonstrated decreased deformability. Particularly, in 17-25 age group, deformability is 4,5 ±0,3, in 25-35 age group 4,8 ±0,4, in older age group, this indicator is decreased 3,0 ±0,3. This can be caused by changes in membrane lipid-protein composition in erythrocytes of older people.

SECOND CLASS CHILDREN PHYSICAL DEVELOPMENT INDICATORS

DISCUSSION IN DYNAMICS

The purpose of this work is to analyze the changes that occurred during a year among  the primary class pupils who belong to the practically  healthy groups. In April 2018, physical indicators  were described: height (body length), mass, head and thoracic circulation. We observed  the second grade pupils at the same three Public Schools in Kutaisi, whose age is 7-8 years. The number of researchers who made a thorough analysis are 98 (it was 105 last year). There are 52 girls and 46 boys. The decrease in number was caused by a migration factor.

As it appeared, the increase was mostly clear among two accelerators: height and mass. The thoracic circulation has increased in only 7-year-old boys, and its surroundings are almost unchanged everywhere. Of course, only by this data it is difficult to assume the signs of the signal of the deceleration.  At least 2-3 years of observation will give us the perfect answer.

PERIODIC FEVER WITH APHTHOUS STOMATITIS, PHARYNGITIS,

AND ADENITIS (PFAPA SYNDROME)

The autoinflammatory diseases constitute a family of disorders characterized by aberrant activation of inflammatory pathways in the absence of antigen-directed autoimmunity. Classically, periodic fevers are the common presenting manifestation. The most common of these conditions with no known genetic defect is periodic fever, aphthous stomatitis, pharyngitis, and adenitis (PFAPA) syndrome. The diagnosis of PFAPA is based upon clinical criteria and the exclusion of other causes of recurrent fevers.PFAPA is a self-limited disease with no long-term sequelae. A decision to treat PFAPA is based upon severity and duration of episodes, impact on quality of life, and preference of the child and their family.
